51例原发性肝癌患者并发抑郁及其中医证候临床观察

目的:观察原发性肝癌并发抑郁的发生率及其中医证候分布规律及特点。方法:采用量表评估和访谈调查方法,对51例原发性肝癌患者并发抑郁及其证候进行临床观察与分析。结果:51例原发性肝癌患者中抑郁发生率约76.47%。其中轻度抑郁发生率为41.18%,中度抑郁23.53%,重度抑郁11.76%。证型以肝郁脾虚为主,出现频率约占60.78%,主要症状为胁痛(76.47%)、腹胀(64.71%)、乏力(60.78%)、纳差(50.98%)、消瘦(43.14%)、口干(41.76%)、少寐(37.25%)等。结论:原发性肝癌并发抑郁发病率高,在肝郁证候基础上,脾虚、血瘀、湿困为主要兼夹证候,通过疏肝健脾解郁治法可有效干预,以提高患者生存质量。

Clinical Observation on Incidence of Primary Liver Cancer Complicated With Depression and Its Syndromes of Traditional Chinese Medicine

Objective ：To observe incidence rate of the primary liver cancer complicated with depression and their distribu- tion regularity and characteristics of TCM syndromes. Methods：51 patients of primary liver cancer complicated with de- pression and their syndromes were studied with the method of clinical observation and analysis. Results：Depression inci- dence rate in 51 patients of primary liver cancer was 76.47%, and percent of mild depression, moderate depression, severe depression was 41.18%,23.53%, 11.76% respectively. Liver depression and spleen deficiency type which syndromes in- cluded hypochondriac pain, abdominal distension,fatigue,poor appetite ,weight loss ,dry mouth,less sleep soundly,and so on was the common TCM type in primary liver cancer patients. Conclusion ：The incidence of primary liver cancer compli- cated with depression is high. Spleen deficient, blood stasis and wet trapped are the common types and accompanied symp- toms in the liver depression syndrome. Accordingly, soothing liver and strengthening spleen can effective intervene primary liver cancer complicated with depression and improve patients＇quality of life.
