腹腔镜保留睾丸动脉的精索静脉高位结扎82例术后疗效观察

目的评价腹腔镜保留睾丸动脉的精索静脉高位结扎术的效果及价值。方法回顾性分析2005年10月至2010年10月共82例腹腔镜下保留睾丸动脉的精索静脉高位结扎术的临床资料,其中左侧57例,右侧11例,双侧14例。结果 82例手术均顺利完成,手术时间12～51min,平均21.8min,手术出血极少。术后患者精子密度、精子活力与术前比较有显著提高（P〈0.05）,精子畸形率与术前比较显著降低（P〈0.05）。结论腹腔镜保留睾丸动脉的精索静脉高位结扎术具有疗效可靠、提高精子质量、创伤小、恢复快等优点,可作为有生育要求精索静脉曲张患者的首选治疗方法。

Testic Artery Sparing Laparoscopic Varicocele High Ligation in 82 Cases

Objective To estimate the effect and value of high ligation of spermatic vein of keeping arteriae testicularis by laparoscope.Methods 82 patients who have had the opration from October 2005 to October 2010 are analyzed retrospectively.Among the total,57 patients keep left arteriae testicularis,11 right,and 14 both.Results The operations of the 82 patients are successful.The time of opration is 12～51min,21.8 in average,and there is little blood loss during the opration.After the operation,the sperm count and vitality of patients raise significantly（P0.05）,while the rate of teratospermia are lower than before（P0.05）.Conclusion The opration has the merits of reliable effect,increasing the quality of sperm,little trauma,pleased recovery.It can be the first selection of the patients who have varicocele and desire of breed.
