胃肠间质瘤的CT和MRI诊断价值

目的 分析胃肠间质瘤( GIST)的多层螺旋CT和MRI表现,评价其对该肿瘤的诊断价值.方法 回顾性分析18例经病理证实的胃肠间质瘤的螺旋CT和MRI资料,其中10例行CT及MRI检查,8例只行CT检查.结果 18例GIST发生于胃9例,小肠5例,结肠及直肠4例；邻近组织器官侵犯4例.肿瘤多呈圆形、类圆形或不规则形.7例向腔内生长,11例腔外或同时向腔内外生长；平扫8例密度或信号均匀,10例不均匀,增强扫描7例均匀轻中度强化,11例不均匀明显延迟强化.10例行DWI检查,8例呈高信号,其中4例中心见低信号；2例呈等、低信号.结论 GIST的CT及MRI表现具有一定特征性,多层螺旋CT及MRI检查能准确诊断GIST及其与周围组织关系,对设计治疗方案及预后评估有重要价值.

CT and MRI Diagnosis of Gastrointestinal Stromal Tumor

Objective To evaluate the diagnostic value of MSCT and MRI in the findings and pathologic features of gastrointestinal stromal tumor （GIST）. Methods 18 cases with GIST confirmed by pathology and immunohistochemistry were retrospectively analyzed. 10 patients with CT and MRI, 8 patients only with CT examination. Results In 18 cases, 9 lesions were located in stomach, 5 in small intestine, 4 in colon and rectum. Of 4 cases infringed adjacent tissues and organs. The tumors were round or lobulated, margins were well-defined or poorly-defined; growth pattern： intra-cavity （n =7）, extra-cavity and mixed （n =11）; The signal or density was uniform （n =8）, 10 cases of non-uniform. 8 cases showed high signal, and 4 centers were low signal, 2 cases were so low signal on DWI. Conclusion CT and MRI findings of GIST have a certain characteristic. Multi-slice spiral CT and MRI can accurately diagnose GIST and its relationship with the surrounding tissue, which plays an important role in the design of treatment and prognosis.
