不同脂肪乳剂对危重早产儿脂质过氧化及抗氧化状态的影响

摘    要:目的:观察不同脂肪乳剂对危重早产儿脂质过氧化及氧化状态的影响。方法:将50例患有围生期窒息等疾病的危重早产儿随机分成两组,每组25例。除给予相同的对症、支持治疗外,出生48 h后分别给予含α-维生素E的中/长链脂肪乳剂(MCT/LCT-E组,添加α-维生素E的20%Lipofundin)和长链脂肪乳剂(LCT组,20%Intralipid),连续7 d。另选出生时无窒息、无感染及不需氧疗的早产儿15例作对照组。在试验前和结束时分别检测血浆中MDA、α-tocopherol、维生素C及无机硒浓度。结果:MCT/LCT-E组、LCT组患儿治疗前血浆中α-tocopherol、维生素C及无机硒浓度低于对照组,治疗前血浆中MDA浓度高于对照组。应用脂肪乳剂治疗1周后,LCT组患儿的血浆MDA浓度明显高于MCT/LCT-E组,而LCT组患儿的血浆α-tocopherol及无机硒浓度明显低于MCT/LCT-E组。结论:添加α-维生素E的中/长链脂肪乳剂可降低脂肪乳剂输注所致的脂质过氧化,有效地维持或改善患有围生期窒息等疾病的危重早产儿患儿的抗氧化状态。对于患有围生期窒息等疾病的危重早产儿,应用添加α-维生素E的中/长链脂肪乳剂显著优于长链脂肪乳剂。

Abstract:Objective:To observe the effects of different kinds of fat emulsion on lipid peroxidant and antioxidant status in critically ill preterm infants.Methods:Fifty preterm infants with perinatal asphyxia were randomly divided two groups,and received the same supportive therapy,but different fat emulsions at 48 hours after birth:20% Lipofundin supplemented with α-tocopherol(MCT/LCT-E group,n=25) and 20% Intralipid(LCT group,n=25).Each group received fat emulsion for 7 days.Fifteen preterm infants without perinatal asphyxia,infection diseases and oxygen therapy were served as controls.Plasma malondialdehyde(MDA),α-tocopherol,vitamine C and selenium were measured before and after the observation period.Results:The concentrations of plasma α-tocopherol,vitamine C and selenium were lower both MCT/LCT-E group and LCT group in pre-treatment than those in the controls.The MDA concentrations were significantly higher both MCT/LCT group and LCT group in pre-treatment than those in the controls.After 7 days of fat emulsion infusion,the MDA concentrations were significantly higher in LCT group than those in MCT/LCT-E group,the cokcentrations of plasma α-to copherol and selenium were significantly lower in LCT group than those in MCT/LCT-E group.Conclusion:MCT/LCT emulsions supplemented with α-tocopherol can decrease lipid peroxidation caused by lipid emulsions and effectively maintain and improve anti-oxidation status in critically ill preterm infants with perinatal asphyxia.It is suggested that the MCT/LCT emulsions supplemented with α-tocopherol should be preferably used in preterm infants with perinatal asphyxia of newborn when fat emulsion is indicated.
