不同剂量盐酸氨溴索对治疗早产儿呼吸窘迫综合征肺功能影响的对照研究

摘    要:目的:探讨不同剂量盐酸氨溴索(沐舒坦)对治疗早产儿呼吸窘迫综合征(NRDS)肺功能的影响。方法:选择NRDS患儿63例随机分成治疗组32例和对照组31例,两组在相同综合治疗的基础上,治疗组给予大剂量盐酸氨溴索30mg/(kg·d),分3次用5%葡萄糖注射液10ml稀释后静脉滴注;对照组给予盐酸氨溴索7.5mg/(kg·d),分2次用5%葡萄糖注射液10ml稀释后静脉滴注。观察两组治疗后24、48h呼吸症状改善情况和血气指标变化.比较PaO2、PaCO2及PaO2/FiO2等参数的改变和差异。结果:治疗组32例,经治疗24、48h后,呼吸困难及呻吟明显减轻或消失,血PaO2升高,PaCO2下降,PaO2/FiO2明显增加。治疗组有效率为65.6%,而对照组为32.2%。两组疗效及血气指标、氧合指数均有显著性差异。结论:大剂量盐酸氨溴索对NRDS有较好的疗效,能有效防治NRDS;早期应用可避免气管插管,减少肺表面活性物(PS)应用。

Abstract:Objective: To investigate the different doses of Ambroxol Hydrochloride on the treatment of respiratory distress syndrome (NRDS) of premature children. Methods: 63 cases of children with NRDS were randomly divided into two groups which were the treatment group (32 cases) and the control group (31 cases). The same comprehensive treatment were given in both the two groups. Ambroxol Hydrochloride 30 mg/(kg·d) were divided into 3 times with 5% glucose intravenous drip in the high-dose treatment group, while Mucosolvan 7.5 mg/(kg·d) were divided in twice intravenous drip in the control group. The respiratory symptoms and blood gas indexes were observed after the injection of 24 and 48 hours in both groups, and the value of PaO2, PaCO2 and PaO2/FiO2 were compared between them. Results: In the treatment group, the difficulty in breathing and moaning reduced significantly or disappeared after the treatment of 24 and 48 hours which the blood PaO2 increased, PaCO2 decreased and PaO2/FiO2 increased. The effective rate of the treatment group was 65.6% while the control group was 32.2%. The therapeutic effect and blood-gas values, oxygenation index of the treatment group were improved significantly to the control group. Conclusion: High-dose Ambroxol Hydrochloride have good therapeutic effects so it could be used to the prevention and treatment on NRDS. And its early application could avoid endotracheal intubation and reduce the application of PS.
