硬膜外腔术前注射小剂量吗啡用于混合痔术后镇痛

为观察小剂量吗啡于术前注入骶管腔对混合痔术后镇痛的效果，我们将200例于我院行手术治疗的ASAⅠ、Ⅱ级混合痔患者随机分为对照组（2％利多卡因15ml＋0．75％罗哌卡因4ml＋生理盐水6m1）和吗啡组（吗啡2mg，其余药物及剂量同对照组），进行临床对比观察。结果显示，术后4h的VAS评分两组间差异无统计学意义，P〉0．05；而术后8h、24h、48h、72hVAS评分两组间差异均有统计学意义，P〈0．05。吗啡组患者不良反应发生率明显高于对照组，P〈0．05。结果表明，小剂量吗啡同局麻药一起术前注入骶管腔用于混合痔术后患者，可有效镇痛。

Application of Extradural Injection of Small Dosage of Morphine in the Postoperative Analgesia of Mixed Hemorrhoid

For observing the efficacy of small-dosage of morphine injected into sacral canal for postoperative analgesia,the two hundreds patients with mixed hemorrhoids（ASA Ⅰ and Ⅱ ） were randomized into morphine group（patients received preoperative injection of morphine into the sacral canal, i. e morphine 2mg＋2% Lidocaine 15ml＋0. 75% Ropivacaine 4ml＋NS 6ml） and control group（2% Lidocaine 15ml＋ 0.75 % Ropivacaine 4ml＋NS 6ml）. As results, there was no statistical significance in VAS score between the two groups at 4th hour postoperatively（ P 〉0.05） ;while there was statistical significance at the 8th, 24th,48th and 72nd hour VAS score between the two groups （ P 〈0.05） ;and the incidence rate of adverse reaction in the morphine group was obviously higher than that in the control group （ P 〈0.05）. It is concluded that an effective analgesia can be obtained by preoperative injection of small dosage of morphine and local narcotic into the sacral canal.
