两种内括约肌切断术治疗慢性肛裂286例疗效比较

为观察两种内括约肌切断术治疗慢性肛裂的效果，将286例肛裂患者分为两组，I组采用肛裂切除术加后位内括约肌切断术．Ⅱ组采用侧方皮下内括约肌切断术进行治疗。结果显示，两组近期均治愈，I组术中出血较多，创口愈合慢，但肛门括约肌松解彻底，无复发，术后并发症少；Ⅱ组术中出血较少，创口愈合快，但易形成血肿，易复发。结果表明，对慢性肛裂患者行内括约肌切断术治疗应视其具体情况而定，术式选择应个体化。

Comparison on the Therapeutic Effects of Two Inner-sphincterotomy for Chronic Anal Fissure of 286 Cases

The objective of this study was to observe the therapeutic effects of two inner-sphincterotomy for chronic anal fissure. The 286 patients with anal fissure were divided into two groups： I and Ⅱ group. I group received the excision of anal fissure and posterior inner-sphincterotomy,while Ⅱ group was subjected to lateral hypodermatic inner-sphincterotomy. As results,in shorter-term both groups gained curative outcome;in I group bleeding volume was more,wound healing was slower, but solution of anal sphincter was throught and no recurrence was found,and postoperative complication also was less;while in II group bleeding volume was less,wound healing was faster, but hematoma was easily formed and recurrence easily happened. Above outcomes indicate that procedure selection should be taken according to the principle of personality,performing inner-sphincterotomy must depend on the condition of patient.
