药线拖线法与瘘管切开法治疗复杂性肛瘘的疗效比较

为评价药线拖线法治疗复杂性肛瘘的疗效和安全性，将92例复杂性肛瘘患者分别采用药线拖线法（治疗组）和瘘管切开法（对照组）治疗。以愈合时间、临床症状等作为主要的观察指标。结果显示，治疗组平均治愈时间为（27．3±6．3）d，平均支管愈合时间为（16．4±4．6）d；对照组分别为（37．0±7．2）d和（23．0±8．7）d，差异均有统计学意义（P〈0．01）。治疗组46例均未出现肛门畸形、肛门失禁等并发症或后遗症。对照组有11例出现不同程度的肛门周围凹陷性瘢痕，肛门对肠气、肠液、稀便不能控制等后遗症。结果表明，药线拖线法治疗复杂性肛瘘能保护肛管括约肌功能，缩短疗程，提高患者生活质量。

Comparison of the Therapeutic Efficacy between the Procedure of Drawing of Medicated Thread in Fistula and the Procedure of Fistulotomy in the Treatment of Complicated Anal Fistulae

Ninety two patients with complicated anal fistulae were evenly divided into study group and con- trolled group. Patients in the study group underwent the procedure of drawing of medicated thread in fistula,while patients in the controlled group underwent fistulotomy. As results, the average healing time and the average healing time of the fistula branches was （27.3±6.3）days and （16.4±4.6）days in the study group,and that was （37.0±7.2）days and （23.0 ± 8.7）days in the controlled group, there was statistical significance（ P 〈0.01）. There was no such complications or sequelae as anal deformity and fecal incontinence in the study group, while perianal concave scar and incontinence of intestinal gas, liquid and loose stool occurred in 11 cases in the controlled group. It is concluded that the procedure of drawing of medicated thread in the fistula in the treatment of complicated anal fistulae has the advantages of protection of anal sphinctr,reduction of treatment course and enhancement of life quality.
