颈动脉硬化与脑梗塞的相关性探讨

目的探讨脑梗塞与颈动脉粥样硬化的关系。方法对60例经CT或MRI证实的脑梗塞患者和60例非脑梗塞患者行彩色多普勒超声检查,观察颈动脉内壁光滑程度和有无动脉粥样硬化。结果60例脑梗塞患者中,43例有不同程度的颈动脉斑块形成,发生率71.7%;非脑梗塞组发生率36.7%,两组间有显著性差异(P<0.01)。脑梗塞病例中有25例为多灶性脑梗塞,颈动脉粥样斑块发生率为80.0%;单一病灶的脑梗塞35例,颈动脉粥样斑块发生率为65.7%,二者比较差异有显著性(P<0.05)。在60例脑梗塞患者中首次发生脑梗塞39例,颈动脉粥样斑块发生率为81.0%;复发性脑梗塞21例,颈动脉粥样斑块发生率为67.7%,二者经统计学处理有显著差异(P<0.05)。结论颈动脉粥样硬化与脑梗塞发生率、脑梗塞病灶多寡以及复发有密切的相关性。

To Evaluate the Relationship Between Between Carotid Arteriosclerosis and Cerebral Infarction

Objective To study the relationship between carotid arteriosclerosis(CAS) and cerebral infarc- tion with color doppler ultrasonography. Methods All 60 patients with cerebral infarction and 60 patients without cerebral infarction diagnosed by CT or MRI were examined in the alternations of carotid dimension ,intramemebrane morphology and hemodynamics with color doppler ultrsonography. Results The incidence of carotid plaques were 71.7% and 36.7% respectively in infarct group and control group,the difference was significantly. The incidence of carotid plaques were increased significantly in case of multi-foci infarction and recurrent infarction.Conclusion CAS can increase the incidence of cerebral infarction .CAS have positive correlation with multi-foci infarction and recurrent infarction.and cerebral infarctionare closely related. CAS can be diagnosed in early period by ultrasonography ,which is important to pretend the onset of cerebral infarction.
