肝切除术治疗肝胆管结石的疗效观察

目的:观察应用肝切除术治疗肝胆管结石的疗效。方法:在135例手术治疗的肝胆管结石病人中,61例(45.2%)采用了肝切除术(研究组),74例术中未采用肝切除术(对照组)。比较两组的疗效和并发症的发生率。结果:研究组治愈率(98.4%)高于对照组(85.1%)(P<0.05),残石率(9.8%)低于对照组(29.7%)(P<0.01),而两组的手术并发症并无显著差异(P>0.05)。研究组术后2年和5年随访的疗效优良率(分别为94.2%和89.1%)显著高于对照组(分别为73.3%和68.6%)(P<0.01和P<0.05)。结论:应用肝切除术治疗肝胆管结石的疗效好于对照组病例。

Effects of hepatectomy in the treatment of hepatolithiasis

Objective To observe the effects of hepatectomy in the treatment of hepatolithiasis. Methods Of 135 patients with hepatolithiasis, 61(45.2%) were submitted to hepatectomy (study group), and 74 patients underwent other surgical procedures without hepatectomy (control group). The results of treatment were compared between the 2 groups. Results Statistically, the study group had higher complete relief rate (98.4%) and lower residual stone rate (9.8%) than the control group (85.1% and 29.7%, respectively) (P<0.05 and P<0.01, respectively), although the rate of postoperative complications was similar in the 2 groups (P>0.05). The results of 2 and 5 years follow-up showed that, the percentage of patients with excellent and good outcome was significantly higher in the study group (94.2% and 89.1%) than that in the control group (73.3% and 68.6%) (P<0.01and P<0.05, respectively). Conclusions Application of hepatectomy in the treatment of hepatolithiasis offers better results than the control group.
