双额脑严重挫裂伤的外科治疗

目的:探讨超早期双额开颅减压术治疗双额严重脑挫裂伤的疗效。方法:我科近8年来收治53例严重双额脑挫裂伤患者,其中27例在入院后6小时内行双额开颅减压术,另26例入院后先行保守治疗,对两组的疗效进行比较。结果:伤后3个月按GOS评定,超早期手术组的优良率81.5％,死亡率为3.7％,精神异常的恢复率90％,保守治疗组的优良率为65.4％,死亡率15.4％,精神异常的恢复率53.8％。结论:超早期双额开颅减压术治双额严重脑挫裂伤有较好的疗效。

Surgerical Treatment of Bilateral Frontal Cerebral Contusion

Objective: Discuss the effects of super-early operation on bilateral frontal cerebral contusion. Methodes: 27 of 53 cases with severly bilateral frontal cerebral contusion have been operated by decompression of bilateral frontal lobes with 6 hours, 26 cases were given conservative therapy first, the theraputic results were compared and studied. Results: According to GOS standard, all of the restdts about these patients were assessed 3 months after injury. In early operation group, the good result rate is 81.5%, death rate is 3.7%, mental disorder recovery rate is 90%. In conversative treatment group, good result rate is 65.4%, death rate is 15.4%, the recovery rate of mental disorder is 53.8%. Conclusion: The therapeutic result in early operation group is better markedly than conversative therapeutic group.
