机械吻合和手工吻合在国人食管癌术后吻合口瘘发生率的Meta分析

摘    要:目的对比机械吻合与手工吻合对食管癌术后吻合口瘘发生的概率,为临床选择吻合方式提供参考依据。方法运用Meta分析方法,综合定量分析国内公开发表的有关食管癌患者采用机械吻合与手工吻合对预防食管癌术后吻合口瘘的12篇文献资料。结果采用手工吻合食管癌术后吻合口瘘的发生率高于机械吻合,合并OR值为0.53(>0,95%CI:0.40~0.71,P<0.01 )。结论对于降低食管癌手术后吻合口瘘方面,机械吻合优于手工吻合。

Abstract:ObjectiveTo compare the probability of anastomotic leak in the Chinese patients with carcinoma of esophagus using stapled anastomosis and artificial anastomosis in order to provide some reference for patients with carcinoma of esophagus.MethodsBy Meta—analysis we made a comprehensive analysis of 12 literatures published in China on stapled anastomosis and artificial anastomosis in preventing anastomotic leak in patients with carcinoma of esophagus.ResultsThe probability of anastomotic leak in patients with carcinoma of esophagus was obvious heighten in using artificial anastomosis compared to that in using stapled anastomosis,with the combined odds ratio(OR)value being 0.53(>0,95%CI: 0.40~0.71,P<0.01 ).ConclusionTo prevent anastomotic leak in patients with carcinoma of esophagus, stapled anastomosis is more predominant than artificial anastomose.
