精子核成熟度与早期不明原因复发性流产的相关性研究

目的:探讨精子核成熟度与早期不明原因复发性流产的相关性。方法:收集配偶具有早期不明原因复发性流产史的患者50例为URM组,正常生育的男性30例为对照组,通过计算机辅助精液分析仪检测精子的浓度和活动力等,精子染色质扩散实验检测精子DNA完整性,苯胺蓝染色法检测精子核蛋白组型转换,分别比较两组精子DNA完整性、精子核蛋白组型转换率和精液参数的差异。结果:URM组与对照组之间精液量(3.23±1.13 vs 3.37±1.23)mL、精子浓度(29.33±3.76 vs 31.16±4.29)×10~6/mL的比较差异没有统计学意义(P0.05);精子前向运动(35.19±7.26 vs 50.12±10.84)%、精子正常形态(2.07±2.94 vs 8.69±4.13)%、精子DNA碎片率(39.28±15.95 vs 23.16±15.17)%及精子核蛋白组型转换率(41.99±8.63 vs 18.81±7.53)%比较差异有统计学意义(P0.05)。结论:早期不明原因复发性流产可能与精子核成熟度异常有关,建议RSA患者常规检测夫精精子核成熟度。

Correlation between sperm nuclear maturity and earlyunexplained recurrent miscarriage

Objectives:To investigate the correlation between sperm nuclear maturity and early unexplained recurrent miscarriage (URM).Methods:50 patients who had a history of unexplained recurrent miscarriage were collected as URM group and 30 healthy fertile men as control group.Computer-assisted semen analysis was adopted to detect sperm concentration and motility,and sperm chromatin dispersion test was used for detecting sperm DNA integrity and aniline blue staining sperm nucleoprotein conversion.The sperm DNA integrity,sperm nucleoprotein conversion rates and semen parameters were compared between the two groups.Results:Difference in semen volume (3.23 ± 1.13 vs 3.37 ± 1.23) and sperm concentration (29.33 ± 3.76 vs 31.16 ±4.29) between the two groups was not statistically significant (P ＞0.05).Difference in sperm forward motility (35.19 ± 7.26 vs 50.12±10.84),normal sperm morphology (2.07 ± 2.94 vs 8.69 ± 4.13),sperm DNA fragmentation rate (39.28 ± 15.95 vs 23.16 ± 15.17) and sperm nucleoprotein conversion rate (41.99 ±8.63 vs 18.81 ±7.53) was statistically significant between the two groups (P ＜ 0.05).Conclusion:Early unexplained recurrent miscarriage may be related to abnormal sperm nuclear maturity.Therefore,sperm nuclear maturity in patients with RSA is suggested to test in clinical.
