前列腺癌粒子植入后性功能状况及危险因素分析

目的:探讨前列腺癌粒子植入患者性功能状况及主要影响危险因素。方法:对339例前列腺癌粒子植入患者中术前性功能正常的患者119例,采用国际勃起功能问卷评分(IIEF-5)对性功能正常的119例治疗后性功能状况进行调查,对患者年龄、国际前列腺症状评分(IPSS)、前列腺体积、体重指数(BMI)、是否有糖尿病病史、是否术后雄性激素阻断(ADT)治疗与性功能关注状况进行分析,采用单因素及多因素logistic回归分析患者性功能障碍的主要危险因素。结果:119例患者中,粒子植入后勃起功能障碍32.77%(39/119)。轻度勃起异常的24.37%(29/119)中度勃起功能障碍34.45%(41/119),勃起功能正常的8.4%(10/119),勃起功能障碍的的危险因素包括:年龄、糖尿病、BMI、性生活关注、前列腺体积、IPSS评分、ADT。其中术后ADT、年龄、IPSS评分、前列腺体积是主要的危险因素。结论:前列腺癌粒子植入后性功能状况的危险因素与患者年龄、糖尿病、ADT、BMI、IPSS评分、前列腺体积有关;年龄、ADT、IPSS评分是勃起功能障碍的主要危险因素。

Risk factors and sexual function of patients with prostate cancer after brachytherapy

Objectives:To study the sexual function and main risk factors of patients with prostate cancer after brachytherapy.Methods:339 patients with prostate cancer were treated by brachytherapy in our hospital,among who 119 patients were with normal sexual function before treatment.The postoperative sexual function of the 119 patients were detected using the International Index of Erectile Function Questionnaire score (IIEF-5).Analysis was conducted in terms of patients age,international prostate symptom score (IPSS),prostate volume,body mass index (BMI),a history of diabetes,androgen deprivation treatment (ADT) and sexual health concerns.Moreover,univariate and multivariate logistic regression analysis was conducted to analyze major risk factors of sexual dysfunction in patients.Results:Among the 119 cases,the incidence of erectile dysfunction,mildly erectile abnormal,moderate erectile dysfunction and normal erectile function was 32.77％ (39/119),24.37％ (29/119),34.45％ (41/119) and 8.4％ (10/119) respectively after brachytherapy.Risk factors of erectile dysfunction included age,diabetes,BMI,sexual concerns,prostate volume,IPSS scores,ADT,wherein ADT,the age,IPSS score,prostate volume were major risk factors.Conclusion:The sexual function status of patients with prostate cancer after brachytherapy is associated with age,diabetes,ADT,BMI,IPSS score and prostate volume,wherein the age,ADT and IPSS score being major risk factors for erectile dysfunction.
