一次性宫颈球囊扩张器在足月妊娠促宫颈成熟及引产中的临床应用价值

目的:探究一次性宫颈球囊扩张器在足月妊娠促宫颈成熟及引产中的可行性及安全性。方法:根据入院顺序将具明显引产指征的148例足月妊娠初产妇随机分为两组,观察组83例引产采用一次性宫颈球囊扩张器,对照组65例引产采用小剂量缩宫素,对比分析两组的宫颈Bishop评分、宫颈成熟率、引产到临产时间、自然分娩率、分娩产程、母婴并发症等指标的差异。结果:与对照组比较,观察组宫颈Bishop评分、宫颈成熟率、顺产率显著升高,引产到临产时间、分娩第一产程及总产程显著缩短,有统计学意义(P0.05);但两组产后出血量、尿潴留、宫颈裂伤、产褥感染、Apgar评分、转NICU率无统计学意义(P0.05)。结论:一次性宫颈球囊扩张器能促进足月妊娠产妇宫颈成熟,提高自然分娩率及分娩安全,值得推广应用。

Clinical application value of disposable cervical balloon dilator in full-term pregnancy cervical ripening and labor induction

Objectives:To explore the feasibility and safety of disposable cervical balloon dilator in full-term pregnancy cervical ripening and labor induction.Methods:148 full-term pregnancy women with apparent labor induction indication were randomly assigned to two groups according to the admission order,the observation group of 83 cases using disposable cervical balloon dilator,and the control group of 65 cases by small dose oxytocin.The difference in cervical Bishop score,cervical ripening rate,induction of labor to labor,natural birth rate,labor process,maternal and neonatal complications between the two groups were compared and analyzed.Results:Compared with the control group,the cervical Bishop score,cervical ripening rate,and birth rate of observation group increased significantly,while time needed from induction of labor to labor,first delivery process and total labor process reduced significantly,all with statistical significance (P ＜ 0.05).Difference between the two groups in the postpartum bleeding,urinary retention,puerperal infection,Apagr score and turn NICU rate was of no statistical significance (P ＞ 0.05).Conclusion:Disposable cervical balloon dilator can promote the cervical ripening of full -term pregnancy women,and increase the rate of natural delivery and safe delivery,which is worth popularization and application.
