激素替代联合局部用药治疗围绝经期女性萎缩性阴道炎中的疗效观察

目的:分析并探讨激素替代联合局部用药治疗围绝经期女性萎缩性阴道炎的临床效果。方法:选取2013年8月至2016年8月在医院接受治疗的围绝经期女性萎缩性阴道炎患者105例为研究对象,随机分为A组(35例)、B组(35例)、C组(35例),分别使用不同的治疗方法,A组替勃龙片2.5mg,1次/d,口服,共用3个月。B组用雌三醇软膏,0.5g,第1周,1次/d,第2周1次/2d,第3周1次/3d,第4周后1次/周,共用3个月。A组、B组加阴道放置甲硝唑泡腾片1片/d。C组仅阴道放置甲硝唑泡腾片1片/d治疗,共用3个月。结果:A组治疗有效率为97.14%,B组治疗有效率为94.29%,C组治疗有效率为77.14%。A组与B组治疗有效率明显高于C组(χ~2=5.72,P0.05;χ~2=5.29,P0.05)。治疗后,三组雌二醇水平和子宫内膜厚度无显著差异(P0.05),A组和B组p H值明显低于C组(t=2.78,P0.05;t=2.67,P0.05)。结论:激素替代联合局部用药治疗围绝经期女性萎缩性阴道炎的临床效果显著,复发率低,值得推广。

Efficacy of hormone replacement therapy combined with topical medication for perimenopausal women with atrophic vaginitis

Objectives:To analyze and discuss the clinical efficacy of hormone replacement therapy combined with topical medication for perimenopausal women with atrophic vaginitis.Methods:105 perimenopausal women with atrophic vaginitis from August 2013to August 2016 were selected and randomly divided into group A (n =35),group B (35 cases) and group C (35 cases).Group A received 2.5mg tibolone tablets orally,once / d.Group B received 0.5g estriol ointment,once / d for the first week,once every other day for the second week,once every two days for the third week and once per week for the rest treatment course.All the three groups were vaginally placed with metronidazole effervescent tablets.All the treatment lasted for three months.Results:The therapeutically effective rate in group A,group B and group C was 97.14％,94.29％ and 77.14％ respectively,that of group A and group B significantly higher than group C (x2 =5.72,P ＜ 0.05;x2 =5.29,P ＜ 0.05).After treatment,difference in the estradiol levels and endometrial thickness among the three groups was not significant (P ＞ 0.05),and the pH value of group A and group B was significantly lower than group C (t =2.78,P ＜ 0.05;t =2.67,P ＜ 0.05).Conclusion:Hormone replacement therapy combined with topical medication is effective in treating perimenopausal women with atrophic vaginitis with low recurrence rate,which is worth promoting.
