HPV感染基因型在宫颈正常组织、宫颈鳞状细胞癌和腺癌组织中的分布情况分析

摘    要:目的:探讨分析HPV感染基因型在宫颈正常组织、宫颈鳞状细胞癌和腺癌组织中的分布情况。方法:选取56例宫颈正常组织、48例宫颈鳞状细胞癌组织、45例宫颈腺癌组织分别记为对照组、鳞癌组和腺癌组,均采用PCR以及基因芯片技术对HPV感染23种基因进行分型检测,观察不同组别中其分布情况,并对各组相关资料进行回顾性分析。结果:鳞癌组HPV感染阳性率(91.67%)和腺癌组(73.33%)均远远高于对照组(10.71%)(P0.05),且腺癌组HPV感染阳性率也显著低于鳞癌组(P0.05);对照组、鳞癌组和腺癌组一重感染、二重感染和多重感染分布情况均存在显著差异(P0.05);对照组6例HPV感染阳性患者中,HPV-43基因型所占比例最高,为62.50%;鳞癌组44例HPV感染阳性患者中,HPV-16和HPV-18基因型所占比例最高,分别为40.57%和19.81%;腺癌组33例HPV感染阳性患者中,HPV-16和HPV-18基因型所占比例最高,分别为29.58%和19.72%。结论:HPV-16和HPV-18基因型感染可能参与宫颈鳞癌和腺癌的发生,且在宫颈癌患者组织中,多存在双重或多重HPV感染。

Abstract:Objectives:To discuss and analyze the distribution of HPV genotypes in the cervical normal tissues,cervical squamous cell carcinoma and adenocarcinoma tissues.Methods:56 cases of cervical normal tissues,48 cases of cervical squamous cell carcinoma tissues and 45 cases of adenocarcinoma tissues were selected and respectively noted as the control group,squamous cell carcinoma group and adenocarcinoma group.All of the tissues were detected for HPV infection of 23 kinds of genes by the PCR and gene chip technology,and the distribution situations of different groups were observed,and the related data in different groups were retrospectively analyzed.Results:The positive rates of HPV infection in the squamous cell carcinoma group (91.67%) and adenocarcinoma group (73.33 %) were far higher than that in the control group (10.71%) (P < 0.05),and the positive rate of HPV infection in the adenocarcinoma group was significantly lower than that in the squamous cell carcinoma group (P < 0.05).There were significant differences in the distribution of a serious infection,double infection and multiple infection among the control group,squamous cell carcinoma group and adenocarcinoma group (P < 0.05).In the 6 cases with HPV infection in the control group the HPV-43 genotype had the highest proportion for 62.50%,and in the 44 patients with HPV infection in the squamous cell carcinoma group the HPV-16 and H-PV-18 gene-type had the highest proportions for 40.57% and 19.81% respectively.In the 33 patients with HPV infection in the adenocarcinoma group the HPV-16 and HPV-18 gene-type had the highest proportions for 29.58% and 19.73% respectively.Conclusion:HPV-16 and HPV-18 gene-type infection may be involved in the occurrence of cervical squamous cell carcinoma and adenocarcinoma,and in patients with cervical cancer,double or multiple HPV infection is common.
