高强度聚焦超声治疗子宫肌瘤171例临床并发症分析及预防处理

目的探讨高强度聚焦超声(high intensity focused ultrasound,HIFU)治疗子宫肌瘤患者的临床并发症发生原因及处理对策。方法回顾性分析黄冈市中心医院2014年11月至2016年12月经HIFU治疗的171例子宫肌瘤患者的临床资料。结果 171例患者中出现血尿2例(1.17%),严重疼痛15例(8.77%),术后膀胱区灼热痛1例(0.58%),术后双下肢麻木2例(1.17%),术后尿潴留2例(1.17%),未出现皮肤灼伤现象。分析引起并发症的相关因素,如治疗焦距、治疗频率、术中镇痛、膀胱充盈度、患者心理等,予以及时调整治疗,均缓解了不适。结论 HIFU治疗子宫肌瘤的并发症,经对症处理,各症状在短时间内消失,无远期影响及不适。及时处理,合理治疗可以预防和控制HIFU并发症的发生。

Analysis of clinical complications and preventive measures of 171 cases of uterine fibroids treated by high intensity focused ultrasound

Objective To analyze the causes and treatment strategy of clinical complications of high intensity focused ultrasound (HIFU) treating patients with uterine fibroids.Methods The clinical data of 171 patients with uterine fibroids treated with HIFU from November 2014 to December 2016 in Huanggang Central Hospital were retrospectively analyzed.Results Among the 171 cases,there were hematuria in 2 cases(1.17%),severe pain in 15 cases(8.77%),postoperative bladder area burning pain in 1 case(0.58%),postoperative bilateral lower limbs numbness in 2 cases(1.17%) and postoperative urinary retention in 2 cases (1.17%),no skin burns.Observe the factors that cause complications,such as treatment focus,treatment frequency,intraoperative analgesia,bladder filling and patient psychology.Timely treatment were given and the discomforts were relieved.Conclusion Through symptomatic treatment,the complications of HIFU in treating uterine fibroids disappeared in a short time with no long-term impact and discomforts.Timely and reasonable treatment can prevent and control the occurrence of HIFU complications.
