植入CRT-D对扩张型心肌病心力衰竭患者生活质量的影响

目的:观察扩张型心肌病(DCM)心力衰竭患者植入三腔起搏除颤器(CRT-D)后生活质量的变化,探讨CRT-D对DCM心力衰竭患者生活质量的影响.方法:选用简短表格(SF-36)对接受CRT-D治疗的40例患者进行问卷调查,患者在手术前晚和术后1周分别填写SF-36调查表,由患者本人在半小时内完成,观察一般健康状况(GH)、生理功能(PF)、生理职能(RP)、躯体疼痛(BP)、精力(VT)、社会功能(SF)、情感职能(RE)、精神健康指标(MH),同时比较手术前后左心室射血分数.结果:术后左心室射血分数较术前增加(P＜0.01);患者整体生活质量计分较术前有明显提高(P＜0.01),对女性的改变程度的GH、RP、RE 3个指标较男性更显著(P＜0.05).结论:CRT-D治疗DCM心力衰竭患者能显著改善患者生活质量,改善客观的心功能指标,女性较男性明显,从社会心理生物医学模式的标准看是DCM治疗的较理想方法.

The influence of quality of life for patients with dilated cardiomyopathy and heart failure by CRT-D

Objective：To observe the change of quality of life in heart failure patients with CRT-D and discuss it′s effect.Methods：There were 40 patients who had under gone successful CRT-D,the world popular Short Form（SF-36） was used to evaluate the QOL of these patients.The observation index include namely general health（GH）,physical functioning（PF）,rolelim-itations due to physical problems（RP）,body pain（BP）,vigour technology（VT）,social function（SF）,role limitations due to emotional problems（RE）,mental health indicators（MH）.The data were collected in the evening befor and 7 days after experiment,Left ventricular ejection fraction was also compared before and after surgery.Results：Postoperative left ventricular ejection fraction increased than that before（P〈0.01）,There were statistica l significance of the deviation of GH,RP,RE scores between the male and the female after the procedure（P〈0.05）.Conclusion：CRT-D can safely and effectively improve quality of life for DCM with CHF patients.
