桡动脉冠状动脉介入术后弹力带加压止血器压迫时间的探讨

目的:探讨经桡动脉路径行冠状动脉介入治疗(TRI)术后使用弹力带加压止血器的适宜压迫时间.方法:采用随机对照研究设计,将299例TRI术后应用弹力带加压型(R型)止血器的患者随机分为两组.观察组152例压迫止血6 h,对照组147例压迫止血8 h.比较两组术后手部肿胀程度及血管并发症情况.结果:观察组患者手部肿胀程度及肢端缺血并发症的发生率均显著低于对照组的患者(P<0.05).两组均出现皮下血肿及切口渗血的血管并发症,但发生率无明显差异(P>0.05).结论:TRI术后6 h撤除止血器,既可减少血管并发症,又可减轻护理人员的工作量,值得临床推广应用.

Optimal pressing time of compressive haemostatic device after transradial coronary intervention.

Objective：To explore the optimal pressing time of compressive haemostatic device after transradial coronary intervention.Method：This study used a randomized controlled trial design.299 patients undergoing TRI,using a compressive haemostatic device（ZONE）,were randomly divided into two groups.One is the 6-hour pressing time group（observation group,n=152）,the other is the 8-hour group（control group,n=152）.The effects on hand swelling and other vascular related complications were compared.Results：The incidence of mile and moderate hand swelling and limb ischemic complications in observation group significantly lower than in control group（P0.05）.The incidence of subcutaneous hematoma and wound bleeding were similar in two groups（P0.05）.Conclusion：It is suggest that drawing the compressive haemostatic device 6 hours after the TRI,which not only reduces the incidence of vascular related complications but also reduce nurses′ workload.
