Syndecan-1和乙酰肝素酶mRNA表达与胃癌转移和预后的关系

摘    要:目的探讨syndecan-1和乙酰肝素酶(heparanase)mRNA在胃癌组织中的表达及其与胃癌转移和预后的关系。方法采用原位杂交方法,检测118例胃癌组织中syndecan-1和乙酰肝素酶mRNA的表达水平。结果118例胃癌组织中syndecan-1mRNA及乙酰肝素酶mRNA的阳性率分别为42.4%和55.9%;syndecan-1 mRNA及乙酰肝素酶mRNA阳性表达率与肿瘤浸润深度(X^2=32.95,P=0.001;X^2=23.19,P=0.001)、脉管侵犯(X^2=46.22,P=0.001;X^2=33.78,P=0.001)、淋巴结转移(X^2=28.62,P=0.001;X^2=25.43,P=0.001)和远处转移(X^2=63.30,P=0.001;X^2=65.76,P=0.001)密切相关;而syndecan-1 mRNA的表达还与肿瘤大小有相关性(X^2=6.25,P=0.012)。Syndecan-1 mRNA表达水平与乙酰肝素酶mRNA表达呈负相关(r=-0.844,P=0.001);Syndecan-1 mRNA低表达病例平均生存时间和5年生存率低于高表达组,乙酰肝素酶mRNA阳性表达病例的平均生存时间和5年生存率均明显低于阴性表达的病例。结论Syndecan-1 mRNA低表达和乙酰肝素酶mRNA高表达参与胃癌的浸润和转移过程,是指导临床治疗及估计预后的有意义指标。

Abstract:Objective To investigate mRNA expression of syndecan-1 and heparanase in gastric carcinoma, and their correlation with the growth-pattern, invasion, metastasis and prognosis of gastric carcinoma. Methods In situ hybridization technique was used to examine mRNA expression of syndecan-1 and heparanase in 118 specimens of gastric carcinoma. Results The positive rates of syndecan-1 mRNA and heparanase mRNA were 42. 4% and 55. 9% , respectively. The expression of syndecan-1 mRNA and heparanase mRNA were related to tumor invasion depth (X2 = 32. 95, P = 0. 001; X2 = 23. 19, P = 0. 001) , vessel invasion (X2 = 46.22,P = 0.001; X2 =33. 78,P =0. 001) , lymph node (X2 =28. 62,P=0. 001; X2 = 25.43,P = 0.001) and distant metastasis ( X2 =63. 30, P = 0. 001; X2 = 65. 76, P = 0. 001 ) , and syndecan-1 mRNA positive expression was related to tumor size ( X2 = 6. 25, P = 0. 012). There was a negative relationship between Syndecan-1 mRNA and heparanase mRNA expression ( r = -0. 844, P = 0. 001). The mean survival time of cases with low expression of syndecan-1 mRNA was significantly shorter than that of cases with high expression ( r = 36. 48, P = 0. 001 ) , and meanwhile, the mean survival time of heparanase mRNA positive cases was significantly shorter than that of cases with negative expression (r = 34.41, P = 0.001). Conclusions The mRNA expression of syndecan-1 and heparanase can predict the invasion and metastasis of gastric carcinoma, and can be used as markers of prognosis of gastric carcinoma.
