注射hCG日血清孕酮/雌二醇比值对体外受精-胚胎移植治疗结局的影响

目的:探讨超促排卵患者注射hCG日血清P/E2比值对体外受精-胚胎移植(IVF-ET)治疗结局的影响。方法:回顾性分析2003年3月~2006年6月339个IVF-ET周期的资料,将P/E2比值分为3组,组1:P/E2<0.5,组2:0.5≤P/E2≤1.0,组3:P/E2>1.0,对3组的实验室结果和临床结局进行分析。结果:注射hCG日P/E2<0.5时,受精率最低(P(0.01),重度卵巢刺激综合征(OHSS)的发生率最高(P(0.01)。0.5≤P/E2≤1.0时,妊娠率最高。P/E2>1.0时,受精率最高,而妊娠率最低(P(0.01)。结论:P/E2比值过低或过高均不利于患者,只有合适的P/E2值既可提高妊娠率,又可防止发生OHSS,在促排卵周期监测该比值有重要的意义。

Impact of serum progesterone/estration ratio on hCG administration day on clinical outcome of in Vitro fertilization- embryo transfer

Objective:To study the impact of serum progesterone /estration ratio on hCG administration day on clinical outcome of in vitro fertilization-embryo transfer.Methods:339 cases of IVF-ET were analyzed retrospectively,which were performed from March 2003 to June 2006 in our reproduction medical center. 339 cases were divided into 3 groups according serum progesterone /estratiol ratio on hCG administration day,group 1:P/E2<0.5,group 2:0.5≤P/E2≤1.0,group 3:P/E2>1.0,and compared their laboratory results and clinical outcomes.Results:When P/E2<0.5,the fertilization rate was lowest(P﹤0.01),the incidence of severe ovarian hyper stimulation syndrome(OHSS) was the hightest(P﹤0.01). The pregnancy rate was the hightest (P﹤0.01) when 0.5≤P/E2≤1.0.When P/E2>1.0,the fertilization rate was hightes,while the pregnancy rate was lowest(P﹤0.01).Conclusion:The optimal P/E2 ratio on hCG administration day can improve the clinical pregnancy rates,and prevent the incidence of severe in in vitro fertilization-embryo transfer. So it is important to monitor P/ E2 ratio during ovarian stimulation cycles.
