高压氧辅助治疗鼻咽癌放射治疗致放射性脑病的临床研究

目的探讨高压氧对放射治疗（放疗）所致放射性脑病的鼻咽癌患者的治疗价值。方法将47例因放疗导致放射性脑病的鼻咽癌患者分为高压氧（HBO）治疗组和对照组，HBO治疗组采用药物＋高压氧治疗，对照组采用单纯药物治疗。治疗前、后比较2组患者的临床症状、病灶体积和日常生活活动（ADL）能力，并采用Х^2检验和t检验进行统计学分析。结果HBO治疗组临床总有效率达70．0％，而对照组为35．3％，2组比较差异有统计学意义（P〈0．01）。治疗后，HBO治疗组病灶体积较治疗前明显缩小（P〈0．01）；2组治疗后病灶体积比较，HBO治疗组明显小于对照组（P〈0．05）。2组治疗前、后ADL能力比较，差异均有统计学意义（P〈0．05）。结论高压氧结合药物是治疗鼻咽癌放射治疗所致放射性脑病行之有效的手段之一。

The effects of hyperbaric oxygen on patients with radiation encephalopathy caused by the irradiation of nasopharyngeal carcinoma

Objective To evaluate the therapeutic effects of hyperbaric oxygen ( HBO) on patients with radiation encephalopalhy (RE) caused by the irradiation of nasopharyngeal carcinoma (INC). Methods Forty-seven RE cases were divided into a HBO group and a control group. All patients were treated with drugs, in addition to HBO treatment in the HBO group. The therapeutic effects were evaluated in terms of clinical signs, lesion volumes as measured by MRI, and abilities in activities of daily living ( ADL) scores. Results The total efficiency rate was 70% in the HBO group and 35. 3% in the control group. Lesion volumes were significantly decreased in the HBO group after the treatment compared with the control group. There was a significant difference in ADL scores in both groups before and after treatment. Conclusion HBO combined with drug treatment is an effective treatment for RE caused by INC.
