精神心理疗法在脑卒中患者中的临床应用效果观察

目的：探讨精神心理疗法在脑卒中患者中的临床应用效果，评估心理康复治疗对脑卒中后抑郁症的治疗效果。方法选取2011年8月～2013年7月内科住院及内科门诊脑卒中后抑郁患者共72例作为研究对象，按照随机数字表法分为治疗组（37例）和对照组（35例），对照组常规给予药物治疗和功能锻炼治疗，治疗组在对照组的基础上增加每周3次的精神心理治疗，以汉密尔顿抑郁量表（HAMD）评分以及神经功能缺损评分作为观察指标评估两组的临床效果、并进行比较分析。结果治疗前两组患者的HAMD评分以及神经功能缺损评分间差异均无统计学意义（P＞0.05），治疗后1、3个月，两组患者的HAMD评分以及神经功能缺损评分均显著降低，治疗组降低幅度更大，与对照组比较差异有统计学意义（P＜0.05）。结论精神心理疗法在脑卒中患者中的临床应用效果满意，能够更好地改善患者的抑郁及神经功能缺损状况，促进患者的康复，提高患者生存质量。

Clinical effect observation of stroke patients with mental and psychological therapy

Objective To explore clinical effect of Stroke patients with mental and psychological therapy,and assess the psychological rehabilitation of depression after stroke treatment. Methods 72 patients with poststroke depression from Aug. 2011 to Jul. 2013 in our hospital and internal department were devided into two groups by means of random number table. The control group （37 cases） was given medication and functional exercise.Treatment group（37 cases） increased in the control group on the basis of mental and psychological therapy 3 times a week,Hamilton Depression Scale （HAMD） and Neurologic impairment score of the two groups of patients before treatment and after treatment scores to determine clinical efficacy. Results The two groups in HAMD scores and Neurologic impairment score before treatment were no significant differences （P〉0.05）.The two groups in HAMD score and Neurologic impairment score after 1 and 3 months were reduced than before treatment;The treatment group were significantly lower than control group （P 〈 0.05）. Conclusion Mental and psychological therapy in patients with Stroke patients and its clinical application effect of satisfaction, can improve the patient＇s depression and neurologic conditions, promote the rehabilitation of the patient and improve the patients quality of life.
