剖胸术后心律失常的临床分析及预防

目的:分析剖胸术后心律失常的诱发因素,探讨综合预防措施。方法:对61例剖胸术后心律失常按不同手术类型、年龄分布、手术前后心电图正常与否等方面分组进行统计分析。结果:剖胸术后心律失常发生率为9.81％(61/622);其中肺切除术者发生率为23.38％(36/154),而尤以单侧肺全切除术者更为明显占45.45％(5/11);年龄分布60岁以上、下者发生率分别为14.63％(43/294)和5.49％(18/328);手术前心电图正常与异常者术后发生率分别为7.81％(42/538)和22.62％(19/84);各分组统计均有极显著差异性;剖胸术后心律失常年代分布有逐年下降的趋势。结论:通过分析,针对性地提出综合预防措施,明显地降低了剖胸术后心律失常的发生率。

Analysis and Prevention of Arrhythmia Induced by Thoracotomy

Objective:To analyse the inductive factors of arrhythmia after thoracotomy and to investigate its comprehensive preventive measures. Methods:61 cases of arrhythmia were divided into groups and statistical analysis was made of different types of operations they had, their ages and pre - and - post operative electrocardiograms. Results: The incidence rate of arrhythmia after surgery was 9.81%(61/622):23.38%(36/154)in patients undergoing pneu-monectomy,45.45%(5/ll)in patients having unilateral pneumonectomy, 14.63%(43/294) in patients over 60, 5.49% (18/328)in patients under 60, 7. 81 % (42/538)in patients with preoperative normal electorcardiograms, and 22, 62% (19/84) in patients with preoperative abnormal electrocardiograms. The statistical analysis showed that there was a significant difference, and the incidence of this complication was decreasing year by year. Conclusion: The incidence of arrhythmia could be greatly reduced after inductive factors were made and preventive measures taken.
