颈部动脉超声在短暂性脑缺血发作中的诊断价值

目的评价颈部动脉超声在诊断短暂性脑缺血发作（TIA）中的应用价值。方法选取2011年1月—2013年8月来该院就诊并确诊为TIA的患者76例作为观察组,选取同期来该院就诊的非心脑血管疾病患者76例作为对照组,应用彩色多普勒超声检测二组患者颈动脉病变情况,具体对照指标有内膜增厚、管腔狭窄、颈动脉血流动力学及粥样斑块检出情况等。结果超声对于观察组颈动脉病变检出阳性率（88.2%）大于对照组（22.4%）,颈动脉扭曲情况（19例）高于对照组（2例）,椎动脉流速低于对照组,斑块检出率也明显高于对照组,差异比较均具有统计学意义（P〈0.05）。结论颈部动脉超声作为一种无创、简便的评价颈动脉粥样硬化和狭窄程度的方法,对于短暂性脑缺血发作有较高的辅助诊断价值。

THE VALUE OF CAROTID ARTERY ULTRASOUND IN THE DIAGNOSIS OF TRANSIENT ISCHEMIC ATTACK

Objective To evaluate the application value of carotid artery ultrasound in the the diagnosis of transient ischemic attack（TIA）.Methods Totally 76 patients with TIA in author＇s hospital from January2012 to August 2013 were selected as the experimental group,and 76 cases of non-cardiac and cerebral vascular diseases were chosen from the same period as the control group.Color Doppler ultrasound was used in two groups of patients with carotid atherosclerosis,the specific body control indexes like intimal thickening,lumen stenosis,carotid artery hemodynamics and atheromatous plaques were tested.ResultsThe positive rate detected through ultrasound for carotid artery disease in experimental group（88.2%）was higher than that of the control group（22.4%）,among which patients with carotid artery twist（19 cases）was higher than that of the control group（2 cases）,vertebral artery flow rate was lower than the control group,and the detection rate of plaque was significantly higher than that in control group.All the differences were statistically significant（P〈0.05）.Conclusion Carotid artery ultrasonography,as a noninvasive,simple method for evaluation of carotid atherosclerosis and stenosis,is of diagnostic value for high transient ischemic attack.
