颈动脉粥样斑块与心脑血管事件临床观察

目的通过对颈动脉内中膜厚度（IMT）和斑块的测定，观察颈动脉粥样斑块与心脑血管事件的相关性。方法采用彩色多普勒测定颈动脉IMT和硬化斑块指数及积分。检测155例患者，脑梗死组56例，心肌梗死组43例，心肌梗死并发脑梗死组24例，同时检测40例无明显心脑血管病患者。结果相关参数对照组与其他3个观察组相比IMT显著低（P〈0．01），3个观察组间的IMT相比P〉0．05；观察组的IMT明显高于正常（P〈0．01），3个观察组间的颈动脉斑块检出率与对照组相比P〈0．01，3个观察组中心肌梗死并发脑梗死组的检出率最高，但三组间比较P〉0．05；3个观察组的斑块指数及斑块积分高于对照组（P〈0．01），心肌梗死并发脑梗死组高于心肌梗死和脑梗死组（P〈0．01），脑梗死组与心肌梗死组相比P〉0．05。结论颈动脉斑块是反映动脉粥样硬化的较好指标，脉斑块与颈动心脑血管事件正相关。

CLINICAL OBSERVATION OF CAROTID ARTERY PLAQUE AND CARDIOVASCULAR AND CEREBROVASCULAR EVENTS

Objective To observe the correlation of carotid artery atherosclerosis and cardiovascular events through the carotid artery intimamedia thickness （IMT） assay and plaque. Methods Carotid IMT and 151aque index and integral were determined by color Doppler. Totally 155 patients received the test, among them 56 patients with cerebral infarction, 43 cases of myocardial infarction, 16 cases of myocardial infarction complicated with cerebral infarction, 40 cases of patients with no apparentcardiovascular disease at the same time. Results The IMT were significantly lower in control group than those in other 3 observation groups, P〈0.01. The detection rate of carotid artery plaque in observation group was significantly higher than that in control group, P〈0.01. Myocardial infarction complicated with cerebral infarction group was higher than that of myocardial infarction and cerebral infarction group, P〈0.01, the difference of Cerebral infarction and myocardial infarction group was no significant, P〈0.05. Conclusion The plaque of carotid artery atherosclerosis is a better index to reflect pulse plaque and is correlation with carotid cardiovascular event.
