影响急诊医患关系相关危险因素分析

目的探讨影响急诊医患关系的相关危险因素。方法对2007年5月—2011年5月于本院急诊科18岁以上就诊的患者,采用门诊患者满意度调查量表进行问卷调查,并对影响医患关系的危险因素进行分析。结果共324名患者的调查问卷被纳入分析,其中投诉组(实验组)患者68名,非投诉组(对照组)患者256名。通过对各影响因素进行多因素Lo-gistic回归分析显示药物选择是否合理(P=0.000;OR=4.066)、药物的价格是否合理(P=0.002;OR=2.941)、是否认真倾听患者陈述(P=0.000;OR=4.466)、治疗方案选择是否征求患者意见(P=0.014;OR=4.736)、医护人员讲话的表情是否良好(P=0.000;OR=8.715)为影响医患关系的独立危险因素。结论药物选择是否合理、药物的价格是否合理、是否认真倾听患者陈述、治疗方案选择是否征求患者意见、医护人员讲话的表情是否良好是影响医患关系的独立危险因素,在急诊科日常医疗工作中应加以重视。

Analysis on Influence Factors of Doctor-patient Relationship in Emergency Department

Objective To investigate the risk factors of doctor-patient relationship in emergency department.Methods From May 2007 to May 2011,a survey of the risk factors of doctor-patient relationship was conducted in patients visited department of emergency with a satisfaction questionnaire of the emergency department.Results 324 patients were included in our analysis and 68 were assigned to complaints group.Multivariate logistic regression analysis revealed that the appropriate choice of the drug(P=0.000;OR=4.066),reasonable price of the drug(P=0.002;OR=2.941),earnestly heed the statement of the patient(P=0.000;OR=4.466),solicited comments of the treatment program from the patient(P=0.014;OR=4.736),and good emotional expression of medical staff(P=0.000;OR=8.715) were the independent influencing factors for doctor-patient relationship.Conclusion The appropriate choice of the drug,reasonable price of the drug,earnestly heed the statement of the patient,solicited comments of the treatment program from the patient,and good emotional expression of medical staff were the independent influencing factors for doctor-patient relationship.We should pay more attention to these risk factors in our medical work.
