以上臂皮神经及其伴行血管为蒂岛状皮瓣的应用解剖

目的 :为上臂皮神经及其伴行血管为蒂岛状皮瓣提供解剖学基础。方法 :在 2 8侧经红色乳胶灌注的成人上肢标本上 ,对上臂皮神经及伴行血管的起源、走行、分支及外径进行解剖观测 ,并研究营养血管与皮肤血供的关系。结果 :上臂所有皮神经均有伴行营养血管 ,其伴行血管在其神经束间及神经旁相互吻合构成丰富的血管网 ,并借分支与筋膜皮支所形成的皮下筋膜血管网沟通 ,营养筋膜皮肤。结论 :以上述 4条皮神经及其伴行血管为蒂 ,可分别设计顺行或逆行岛状皮瓣或游离皮瓣 ,修复临近部位、手部的软组织缺损。

Applied Anatomy Study of Island Flap Pedicled with Cutaneous Nerve of Upper Arm and Its Concomitant Vessels

Objective: To provide the applied anatomic basis for the application of island flap pedicled with the cutaneous nerve of arm and its concomitant vessels. Methods: The origin, course, distribution, anastomosis and out diameter of cutaneous nerve of brachial and its concomitant vessels were observed on 28 sides adult cadaver specimens perfused with red latex.The relationship between nutrient vessels and the range of blood supply of skin was observed and determined. Results: All cutaneous nerve of arm had its concomitant vessels. the concomitant vessels anastomosed with its nerve branch which formed rich vessel network among nerve tract or beside the nerve and connected with brachial vessel network by its branch. Conclusion: The antegrade or retrograde island flap or free flap could be designed by the four cutaneous nerves and their nutrient vessels to the adjacent and hand soft tissue defect.
