中西医结合治疗肝炎后肝硬化肝性胸水32例疗效观察

目的：观察中西医结合治疗肝炎后肝硬化肝性胸水的临床疗效。方法：将62例患者随机分为2组。治疗组32例在常规西医综合治疗基础上加服四妙回春汤（基本方：生黄芪、鳖甲、葶苈子、紫菀、车前子、大腹皮、桔梗、茯苓皮、白术、泽兰、蜀羊泉、大枣）治疗；对照组30例以常规西医综合治疗。2周为1疗程，连续治疗1～3疗程。观察2组临床疗效及复发情况。结果：治疗组显效14例，有效17例，无效1例，总有效率96．9％；对照组显效7例，有效15例，无效8例，总有效率73．3％。2组总有效率比较，差异有非常显著性意义（P〈0．01）。治疗组随访30例，复发4例，复发率13．3％；对照组随访22例，复发10例，复发率45．5％。2组复发率比较，差异有显著性意义（P〈0．05）。结论：中西医结合疗法治疗肝炎后肝硬化肝性胸水疗效满意，复发率低。

Treatment of Cirrhosis Hepatic Hydrothorax After Hepatitis by TCM Combined with Western Medicine: A Clinical Observation of 32 Cases

Objective: To observe the curative effect of TCM combined with western medicine (WM) for cirrhosis hepatic hy- drothorax (CHH) after hepatitis. Methods: 62 cases were allocated to 2 groups randomly. The treatment group (32 cases) was treated by routine WM, and added with Simiao Huichun Tang (composed of Radix Astragali, Carapax Trionycis, Semen Lepidii seu Descuraini- ae, Radix et Rhizoma Asteris, Semen Plataginisl, Pericarpium Are- cae, Radix Platycodonis, Exodermis Poriae, Rhizoma Atractylodis Macrocephalae, Herba Lycopi and Fructus Jujubae), and control group (30 cases) by routine WM alone. 2 weeks constituted a course of treatment and 1-3 courses were given continuously. The clinical curative effect and recurrent situation were ob- served. Results: In treatment group, 14 cases were markedly effec- tive, 17 cases effective and 1 case ineffective, the total effective rate was 96. 9% ; and 7, 15, 8 and 73. 3% in control group respectively ( P < 0. 01). In treatment group, a follow - up in 30 cases, 4 cases were recurred (13. 3% ); in control group, a follow-up in 22 cas- es, 10 cases were recurred (45.5% ), the comparison between 2 groups showed marked significance( P < 0. 05) . Conclusion: The therapy of TCM combined with WM possesses a satisfactory curative effect for CHH after hepatitis.
