血清25-羟基维生素D检测在慢性心力衰竭患者心功能分级评价中的作用

目的探讨血清25-羟基维生素D浓度在诊断慢性心力衰竭(CHF)及心功能分级评价中的作用。方法选107例CHF患者,采用彩色多普勒超声心动图仪测定左室射血分数(LVEF),依据纽约心脏病协会(NYHA)分级方案对CHF患者进行心功能分级。取血测定血清25-羟基维生素D及血浆脑钠肽(BNP)水平,观察血清25-羟基维生素D与BNP及LVEF的相关性。结果 107例慢性心力衰竭患者血清25-羟基维生素D水平与血浆BNP浓度呈负相关(r=-0.424,P<0.01),与LVEF呈正相关(r=0.421,P<0.01)。结论 CHF患者血清25-羟基维生素D水平随着心力衰竭严重程度的增加而降低,血清25-羟基维生素D水平可以准确评价心功能,可作为临床诊断、疗效观察以及预后判断CHF的客观指标。

The role of detection serum 25 - hydroxy vitamin D in the cardiac function grading and evaluation of chronic heart failure patients.

Objective To explore the role of detection serum 25 - hydroxy vitamin D in the cardiac function grading and evaluation of chronic heart failure patients （CHF）. Methods 107 CI-IF patients were included into this study. All these patients diagnosis and grading was ac- cording to New York Heart Association （NYHA） grading scheme. The LVEF was determined by colorful ultrasonic cardiogram. The serum 25 - hydroxyitamin D and plasma brain natriuretic peptide （BNP） levels were detected. The correlation was observed between serum 25 -hydroxyitamin D, plasma brain natriuretic peptide （BNP） and LVEF. Results The serum 25 - hydroxyitamin D level of 107 CHF patients decreased with the CHF grade increasing. The relationship of serum 25 - hydruxyitamin D level and plasma BNP was negative correlationship （ r = - O. 424, P 〈 O. 01 ）. The relationship of serum 25 - hydroxyitamin D level and LVEF was positive correlationship （ r = - 0. 424, P 〈 0.01 ）. Conclusion The se- rum 25 - hydroxyitamin D level can evaluate correctly the heart function, which can be considered as clinical diagnosis and prognostic judgment of CHF objective indicator.
