摘    要:目的研究不同RhD基因型与抗-D产生的相关性,以发现与产生抗-D及能引起Rh血型新生儿溶血病(HDN)相关的D基因表型。方法应用血清学方法结合分子生物学(PCR-SSP)技术。结果 RhD初筛阴性样本128例,再行RhD阴性确认试验,有3例样本阳性,为D变异型。根据分子生物学结果表明缺失RHD基因104例;DEL型21例;弱D15型2例;DⅥ型Ⅲ型1例。结论应对RhD抗原初筛试验阴性的个体进一步鉴定RHD基因型,以预测与评估产生抗-D的可能性,从而对孕产妇进行孕期指导、监测及产后干预。

Research on RhD Genotype of Maternity with RhD Negative

Abstract:Objective To study the relationship between the different RhD genotypes and anti -D antibody production,to discover D genotype that can produce anti D antibody and cause RhD-HDN. Methods Serology method and PCR- SSP technology were used to identify the D genotype. Results 128 patients with RhD screening-negative were detected for RhD negative confirmatory test. 3 cases were positive by indirect Coombs test and convicted to be D variant. According to the PCR reaction results, 104 cases were deletion of the RHD gene,21 cases were DEL, 2 cases were weak DIS, 1 case was D VI type Ⅲ . Conclusions We should further identify the D genotype with RhD antigen negative ,so that we can predict and evaluate the possibility of anti-D antibody production . There will be very important clinical significance for guiding,monitoring or interventing maternity during the pregnancy process or postpartum.
