HnRNP A2/B1在系统性红斑狼疮患者中表达的临床观察

目的探讨系统性红斑狼疮以及狼疮性肾炎患者血清核不均一性核糖核蛋白A2/B1(hnRNP A2/B1)的水平。方法系统性红斑狼疮患者29例设为狼疮组,31例慢性肾小球肾炎患者设为慢性肾炎组,比较两组患者血清hnRNP A2/B1水平。按照C3、C4,anti-DNA抗体阴性或阳性将狼疮组患者分成两组,对两组患者血清hnRNP A2/B1进行t检验。对狼疮患者蛋白尿的量以及病情活动程度进行赋值和血清HnRNP A2/B1水平进行相关性研究。结果狼疮组与慢性肾炎组患者血清hnRNP A2/B1浓度比较差异无显著性(t=-0.24,P=0.811);狼疮性肾炎患者与慢性肾炎组患者血清hnRNP A2/B1浓度无显著差异(t=0.327,P=0.748),狼疮患者中dsDNA阳性和阴性的hnRNP A2/B1水平无显著差异(t=0.397 P=0.695);狼疮患者中血清补体水平降低的血清hnRNP A2/B1浓度显著高于补体正常者(t=-2.282,P<0.05)。狼疮患者24 h尿蛋白定量与血清hnRNP A2/B1水平无等级相关性,(Spearman等级的相关系数为0.259,P=0.174);狼疮活动度与血清hnRNP A2/B1水平存在正相关(Spearman等级相关系数为0.431,P<0.05)。结论狼疮活动期患者血清hnRNP A2/B1水平显著提高,其临床价值仍需后续研究进一步阐明。

Role of serum heterogeneous nuclear ribonucleoprotein A2/B1 level in the patients with systemic lupus erythematosus

Objective To study the significance of serum hnRNP A2/B1 concentration in patients with systemic lupus erythematosus（SLE） and lupus nephritis.Methods 29 cases of systemic lupus erythematosus patients and 31 patients with chronic glomerulonephritis in the control group were included into this study.Serum concentration of hnRNP A2/B1 of two groups were compared with Student t-test.The lupus patients were divided into two groups respectively in accordance with the normal and abnormal C3,C4 group,and negative and positive anti DNA antibodies group.The student t-test was used for comparing the data.Spearman＇s rank correlation was performed to evaluate the relationship between proteinuria score,lupus activity and serum concentration of hnRNP A2/B1.Results There were no significant difference in serum concentration of hnRNP A2/B1 between chronic glomerulonephritis group and lupus group（t =-0.24,P = 0.811）,between chronic glomerulonephritis group and lupus nephritis group（t = 0.327,P = 0.478）,and between negative and positive anti-DNA antibodies in lupus group（t = 0.397,P = 0.695）.An elevated serum hnRNP A2/B1 concentration was associated with lower C3 C4 group（t =-20282,P 0.05）.There was no rank correlation between 24 hours urinary protein rank and serum hnRNP A2/B1 level（Spearman rank correlation： 0.259,P = 0.174）.But there was positive rank correlation between lupus activity rank and serum hnRNP A2/B1 level（Spearman rank correlation： 0.431,P 0.05）.Conclusion Increased serum hnRNP A2/B1 level has been associated with disease activity.Further large scale studies are needed to assess the clinical role of serum hnRNP A2/B1 in systemic lupus erythematosus patients.
