带锁髓内针和形状记忆合金内固定治疗上肢长骨干骨折的临床研究

摘    要:目的 对比带锁髓内针和形状记忆合金内固定治疗上肢长骨干骨折的临床疗效.方法 选择2010年1月~2013年1月我院收治的上肢长骨干骨折患者146例,随机分为两组,每组各73例,分别采用行带锁髓内针(髓内针组)和形状记忆合金内固定(记忆合金组)治疗.比较两组临床疗效、手术情况及术后生活质量.结果 记忆合金组手术时间和骨折愈合时间分别为(1.69±0.62)h和(11.42±3.71)d,显著短于髓内针组(P〈0.05);骨不连发生率仅为2.74%,显著低于髓内针组(P〈0.05);痊愈、总有效率分别达到43.84%和93.15%,显著高于髓内针组32.88%和83.56%(P〈0.05);治疗后总评分达到(84.54±11.24)分,显著高于髓内针组(P〉0.05).结论 形状记忆合金内固定治疗上肢长骨干骨折,手术时间短、愈合快,术后伤口感染和骨不连发生率低,疗效显著优于带锁髓内针内固定治疗.

Abstract:Objective To compare the clinical efficacy of the locking intramedullary nail and the shape memory alloy fixation on the treatment of upper extremity long bone fracture. Methods 146 cases with upper extremity long bone fracture from January 2010 to January 2013 in our hospital were randomly divided into two groups, each group had 73 cases and treated with the locking intramedullary nail or shape memory alloy fixation respectively. The clinical efficacy, operation and postoperative quality of life between two groups were compared. Results The operation time and the healing time of the memory alloy groups were( 1.69 ± 0.62) h and( 11.42±3.71 ) d respectively, significantly shorter than that of the intramedullary nail groups(P〈O. 05). The bone nonunion rate of the memory alloy groups was only 2.74%, significantly lower than that of the intramedullary nail groups(P〈0.05). The healed and total effective rate reached 43.84% and 93. 15% respectively, significantly higher than that of the intramedullary nail groups 32.88% and 83.56%(P〈0.05). The life total score of the memory alloy groups reached (84.54±11.24), significantly higher than that of the intramedullary nail groups(P〉0.05). Conclusion The shape memory alloy fixation in the treatment of the upper extremity long bone fracture is super in that the operation time was shorter, healing was quicker, postoperative wound infection and nonunion rate were lower, and curative effect was better than that of the intramedullary nail with lock.
