肝脏CT灌注成像中各项技术研究

目的:研究肝脏CT灌注成像中各项技术要点。材料与方法:回顾性分析36例肝脏CT灌注扫描资料。扫描前做好患者心理辅导、训练呼吸及肠道清洁工作;经肘静脉注射50ml非离子型造影剂,注射速率4.5ml/s;扫描延时设置8s,应用MSCT的"Toggling-table"技术扫描50s;测定感兴趣区的各项灌注参数。结果:36例中27例能够或基本能够满足读取所需的参数。有9例因各种原因无法读取所需的资料,其中3例因无法控制呼吸而导致移动伪影或扫描层面变化,数据采集不准确;2例为肠道气体多,产生伪影,2例为所需层面无法兼顾,所读取资料不完整;1例为扫描中体位移动,出现伪影及层面之间无法衔接;1例为肝癌合并动脉—门静脉瘘,数据偏差较大。结论:肝脏CT灌注扫描时,如能掌握各项操作要点,既能达到提高图像质量和数据的精度,也能达到减少失误的目的。

The Study of Various Techniques About the Hepatic CT Perfusion Imaging

Purpose:To study the various techniques that were used for the heaptic CT perfusion imaging.Materials and Methods:Thirty six cases with liver cancer were retrospectively analyzed.Before the scanning,the psychological assistance as well as the cleaning of the intestines must be done and the aptients should accept the breathing training.50ml of nonionic contrast media was injected through elbow vein in a speed of 4.5m/s,time delay scanning 8s,by using the MSCT's "Toggling-table"technique to scan for 50s.Test the ROi's perfusion parameter.Results:27 out of 36 can at least basically meet the parameter of rading requirement,however,the information of the 9 cases left can not be read because of a variety of reasons.Among those 9 cases,3 cases are as a result of the out-of-control breath,which leads to the moving fake shadow and the changing scanning slices that influenced the accuracy of the data collecting;2 cases were because of the fake shadow be leaded by the large amount of gas in the enteric cavity;2 cases were owing to the required slices can not be concurred so that the data were not completed;1 case is by reason of the unconnected fake shadow and slices that made by body movement during the scanning that emerge;1 case is because of the big difference of the data that were led by liver cancer combined with A-Portal vein fistula.Conclusion:If the point of the operation can be handled when scanning the hepatic CT perfusion imaging,the quality of the images as well as the accuracy of the data will be improved,mean while,the mistakes can be avoided.
