醒脑安神疏肝解郁针法对脑卒中后抑郁患者神经功能康复的影响研究

目的观察中医醒脑安神疏肝解郁针法治疗脑卒中后抑郁的疗效及对神经功能康复的影响。方法将2017年8月—2018年7月在沧州中西医结合医院治疗的126例脑卒中后抑郁患者随机分为2组各63例,对照组采用盐酸氟西汀胶囊治疗,观察组在对照组治疗基础上辅以醒脑安神疏肝解郁针法治疗,2组均治疗8周,比较2组治疗前后神经功能、抑郁病情、生活质量及血清5-羟色胺(5-HT)、去甲肾上腺素(NE)水平的差异,观察2组治疗效果并记录2组用药安全情况。结果2组治疗后NIHSS评分、HAMD-17评分均明显低于治疗前(P均<0.05),BI指数评分均明显高于治疗前(P均<0.05);且观察组治疗后的NIHSS评分、HAMD-17评分均明显低于对照组(P均<0.05),BI评分明显高于对照组(P<0.05)。2组治疗后血清5-HT、NE水平均明显高于治疗前(P均<0.05),且观察组治疗后的血清5-HT、NE水平明显高于对照组(P均<0.05)。观察组总有效率为92.1%(58/63),高于对照组的74.6%(47/63),差异有统计学意义(P<0.05)。2组均未发生严重药物不良反应。结论对脑卒中后抑郁患者辅以醒脑安神疏肝解郁针法治疗可取得满意疗效,可提高血清5-HT、NE含量,有助于减轻抑郁程度,改善神经功能,提高生活质量,且治疗安全可靠。

Effect of Xingnao-Anshen,Shugan-Jieyu acupuncture on rehabilitation of neurological function in patients with post-stroke depression

Objective It is to observe the therapeutic effect of the traditional Chinese medicine Xingnao-Anshen,Shugan-Jieyu acupuncture method on post-stroke depression and its influence on neurological rehabilitation.Methods 126 patients with post-stroke depression treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from August 2017 to July 2018 were randomly divided into two groups with 63 cases in each group.The control group was treated with fluoxetine hydrochloride capsules,and the observation group was supplemented with Xingnao-Anshen,Shugan-Jieyu acupuncture method,both groups were treated for 8 weeks,the differences in the nerve function,depression,quality of life and serum serotonin(5-HT)and norepinephrine(NE)levels were compared between the two groups before and after treatment,the curative effects and the safety of treatment were evaluated in the two groups.Results After treatment,the NIHSS score and HAMD-17 score of the two groups were significantly lower than those before treatment(P<0.05),and the BI score was significantly higher than that before treatment(P<0.05),the NIHSS score and HAMD-17 score of the observation group after treatment were significantly lower than those of the control group(P<0.05),and the BI scores were significantly higher than those of the control group(P<0.05).The levels of serum 5-HT and NE after treatment in the two groups were significantly higher than those before treatment(P<0.05),and serum 5-HT and NE levels in the observation group after treatment were significantly higher than those in the control group(P<0.05).The total effective rate of the observation group was 92.1%(58/63),which was higher than 74.6%(47/63)of the control group(P<0.05).No serious adverse drug reactions occurred in the two groups.Conclusion Adjuvant therapy with Xingnao-Anshen Shugan-Jieyu acupuncture can achieve satisfactory results,can increase the concentration of serum 5-HT and NE,help to reduce depression,improve neurological function,and improve quality of life in the treatment of post-stroke depression,and the treatment is safe and reliable.
