冠心病合并糖尿病病人出院准备度现状及其影响因素研究

[目的]了解冠心病合并糖尿病病人出院准备度现状及其影响因素。[方法]采用一般资料调查表、出院准备度量表、出院指导质量量表和社会支持评定量表对银川市3所三级甲等医院的239例冠心病合并糖尿病病人进行调查。[结果]冠心病合并糖尿病病人出院准备度总分为(93.77±11.42)分,出院指导质量总分为(139.37±31.84)分,社会支持总分为(35.34±6.79)分,病人的出院准备度与出院指导质量总分和社会支持总分呈正相关(r值分别为0.368和0.200,P均<0.01)。多元线性回归分析显示:感知自理能力、出院指导质量总分、居住方式、工作状态、社会支持总分及服药种类为病人出院准备度的影响因素。[结论]冠心病合并糖尿病病人出院准备度呈中等水平,出院指导质量呈较高水平。医护人员还需改进出院计划,优化出院指导内容,针对感知自理能力较低、独居、社会支持水平低及服药种类较多的病人强化出院指导,以提高病人出院准备度。

Status quo of discharge readiness and its influencing factors in patients with coronary heart disease complicated with diabetes mellitus

Objective:To investigate the status quo and influencing factors of discharge readiness in patients with coronary heart disease complicated with diabetes mellitus.Methods:Totally 239 patients with coronary heart disease complicated with diabetes mellitus in three third grade A hospitals in Yinchuan city were investigated by General Data Questionnaire,Readiness for Hospital Discharge Scale,Quality of Discharge Teaching Scale,and Social Support Rating Scale.Results:The total scores of discharge readiness,quality of discharge guidance,and social support for patients with coronary heart disease complicated with diabetes mellitus were(93.77±11.42),(139.37±31.84),and(35.34±6.79),respectively.Multiple linear regression analysis showed that the self-care ability,the total score of quality of discharge guidance,living style,working status,the total score of social support,and the type of medication were the influencing factors of patients discharge readiness.Conclusions:The discharge readiness of coronary heart disease patients complicated with diabetes mellitus was at the medium level,and the quality of discharge guidance was at higher level.Medical staff should further improve the discharge plan,optimize the discharge guidance content,and strengthen the discharge guidance for patients with lower perceived self-care ability,living alone,with lower level of social support,and with more types of medication,so as to improve the patients′discharge readiness.
