上腔静脉压迫综合征病人经大隐静脉置入PowerPICC SOLO的实践与管理

目的]探讨成人上腔静脉压迫综合征(SVCS)病人超声引导联合心电定位技术下,经大隐静脉留置PowerPICCSOLO导管的临床置管效果及并发症发生率,为此类病人血管通路优选提供依据。方法]为23例SVCS病人在超声引导联合心电定位技术下经大隐静脉留置经外周静脉置入中心静脉导管(PICC),记录病人穿刺时间(从评估血管至完成导管固定)、穿刺次数、穿刺到位率、置管成功率、穿刺时病人疼痛评分。定期维护,跟踪随访,记录病人PICC留置效果及留置期间并发症发生情况。结果]病人穿刺时间为(35.56±3.88)min,穿刺次数为(1.04±0.21)次,穿刺到位成功率为100%,穿刺时疼痛评分为(2.44±0.73)分。所有病人均成功留置PICC。结论]上腔静脉压迫综合征病人超声引导联合心电定位技术下,经大隐静脉置入PowerPICCSOLO安全可行,适用于上腔静脉梗阻病人。

Indwelling of PowerPICC SOLO via saphenous vein in patients with superior vena cava syndrome:the practice and management

Objective:To explore the clinical effect and the incidence of the complications of indwelling PowerPICC SOLO catheter indwelling via saphenous vein under ultrasound guidance combined with electrocardiographic positioning technique in adult patients with superior vena cava syndrome(SVCS),and to provide the basis for preferable vascular access.Methods:A total of 23 patients with SVCS received the central venous catheter(PICC)via saphenous vein under ultrasound guidance combined with electrocardiographic positioning technique through the peripheral saphenous vein.The puncture time(from the vessel evaluation to completion of catheter fixation),puncture times,accuracy rate of puncture,indwelling success rate,and patients′pain score during puncture were recorded.Regular maintenance,follow-up,the patient′s PICC indwelling effect,and complications during indwelling were recorded.Results:The puncture period of the patients was(35.56±3.88)min,the number of punctures was(1.04±0.21)times,the success rate of the puncture in place was 100%,and the pain score during puncture was(2.44±0.73)points.All patients were successfully indwelling PICC.Conclusions:The indwelling of PowerPICC SOLO via saphenous vein under ultrasound guidance combined with electrocardiographic positioning technique in patients with SVCS was safe and feasible,which was suitable for patients with superior vena cava obstruction.
