肝硬化和慢性肝炎病人胃排空及胃动素的变化

目的：研究肝硬化、慢性肝炎病人和正常人空腹血浆胃动素水平及胃窦液体排空功能,方法：应用放免法检测肝硬化42例,慢性乙型肝炎26例,正常人15例（对照组）空腹血浆胃动素,实时超声测量并计算空腹胃窦面积、胃半排空时间及全排空时间。结果：肝硬化组血浆胃动素水平、空腹胃窦面积、胃窦半排空时间均明显高于慢性肝炎组和对照组,而慢性肝炎和对照组之间则无差异。肝硬化病人空腹血浆胃动素及空腹胃窦面积随肝功能损害和加重而增加。结论肝硬化病人存在胃窦液体半排空时间延长、空腹胃窦面积增大,空腹血浆胃动素水平升高,后两者与肝功能损害程度有关。

Alteration of gastric emptying and plasma motilin in patients with liver cirrhosis and chronic hepatitis

Objective To study the levels of fasting plasma motilin and liquid emptying function of gastric antrum in patients with liver cirrhosis,chronic hepatitis and normal adults. Methods The plasma motilin was measured by ra- dioimmunoassay, the area of fasting gastric antrum and the time of gastric emptying were investigated by means of real - time ultrasonography in 42 patients with liver cirrhosis. 26 patients with chronic hepatitis B and 15 normal adults (as control). Results The mean plasma motilin level, antral area and half antral emptying time in liver cirrhosis were high- er than those in chronic hepatitis and control. There was no difference in the latter two groups. The mean antral area and plasma motilin in cirrhosis patients was increased with the severity of the liver functional damage. Conclusion In liver cirrhosis,the half antral liquid emptying time is delayed, fasting antral area is increased and fasting plasma level of motilin is elevated. The antral area as well as plasma motilin is associated with the degree of liver functional damage.
