瘦素可作为诊断多囊卵巢综合征的指标之一

目的 探讨瘦素（Ｌｅｐｔｉｎ）测定在多囊卵巢综合征（ＰＣＯＳ）诊断中的价值。方法 检测３６例ＰＣＯＳ妇女（ＰＣＯＳ组）及２０例正常妇女（对照组）血清瘦素，胰岛素（Ｉｎｓｕｌｉｎ），黄体生成素（ＬＨ）与促卵泡素（ＦＳＨ）的比值，并进行体重指数与Ｌｅｐｔｉｎ的相关性分析和用Ｌｅｐｔｉｎ诊断ＰＣＯＳ时的灵敏度与特异性的ＲＯＣ曲线分析。结果（１）ＰＣＯＳ组血清Ｌｅｐｔｉｎ，Ｉｎ－ｓｕｌｉｎ，ＬＨ／ＰＳＨ比值均高于对照组，差异有显著性（Ｐ＜０．０１）；（２）ＰＣＯＳ组血清 Ｌｅｐｔｉｎ浓度与体重指数（ＢＭＩ）呈显著正相关（ｒ＝０．６１７０，Ｐ＜０．００１）；（３）ＲＯＣ曲线分析显示，血清Ｌｅｐｔｉｎ浓度６ｎｇ／ｍｌ作为诊断ＰＣＯＳ的临界值，其灵敏度达８６．１％，特异性为６５％。结论 血清Ｌｅｐｔｉｎ测定可作为ＰＣＯＳ诊断的参考指标之一。

Value of Leptin in diagnosis of polycystic ovarian syndrome

Objective To explore the clinical value of Leptin in the diagnosis of the polycystic ovarian syndrome (PCOS). Methods The levels of Leptin,insulin,ratio of LH and FSH were detected by RIA and ELISA in serum of 36 cases of PCOS and 20 cases of normal women. Correlative analysis was made between the weight index and the levels of Leptin. Evaluations of Leptin values were made to examine its sensitivity and specificity in the diagnosis of PCOS through ROC curve. Results (1 )The values of Leptin,insulin and LH/FSH in the PCOS group were much higher than those in the control group with statistical difference. (Pr0. 01,P<0. 05). (2)Leptin was increased with the BMI, showing posi- tive correlation (P<0. 01). Line equation y= 1.7947 X--26.4808. (3) ROC curve analysis revealed that when the serum Leptin levels were 4ng/ml, 6ng/ml, 8ng/m1 and 10ng/ml,their sensitivities were 91. 7%, 86. 1 %, 66. 7% and 58. 3% re- spectively; while the specificities were 40%, 65 %,80% and 90% respectively. Conclusion Serum Leptin levels could be served as a diagnostic index for PCOS.
