湖南省肿瘤医院肿瘤病人肺炎链球菌耐药性分析

目的了解我院肿瘤病人临床分离肺炎链球菌的耐药性,为临床合理用药提供依据。方法应用回顾性调查分析方法,对我院2008年1月-2010年12月肿瘤病人临床分离肺炎链球菌对13种常用抗生素的耐药情况进行统计分析。结果 156株肺炎链球菌大部分是从肺癌病人的痰液和咽拭子中分离出来,分离率为79.49%;其次为鼻咽癌病人标本中分离出来,占12.18%;对13种常用抗生素耐药率较低为利福平、米诺环素、庆大霉素,均小于10%;未检出耐万古霉素菌株;耐药率较高为红霉素、阿奇霉素、克林霉素、四环素、克拉霉素、复方新诺明,分别为66.03%、52.56%、51.92%、50%、46.79%、44.87%;多重耐药率为62.18%。青霉素不敏感株（PNSP）发生率为60.26%;PNSP对头孢曲松、阿齐霉素耐药率显著高于青霉素敏感株（PSSP）耐药率,差异均有统计学意义（P〈0.05）。结论我院肿瘤病人肺炎链球菌耐药率有明显上升趋势,临床上应根据药敏结果合理应用抗生素,降低耐药率。
