痰的质量对PCR诊断肺结核的影响

目的评估痰的质量对PCR检查诊断肺结核的影响。方法随机收集2009年3-9月湖南省结核病医院105例确诊为肺结核和22例确诊为非肺结核病人的痰标本,对标本行PCR检测。而这些痰标本中如果有超过40%的活细胞且少于25%的上皮细胞则为高质量标本。结果总痰标本其灵敏度为56.2%,特异度为54.5%,阳性预测值为85.5%,阴性预测值为20.7%,准确度为56.0%。高质量标本的灵敏度74.0%,特异度为60.0%,阳性预测值为94.9%,阴性预测值为18.8%,准确度为72.7%,而低质量标本的灵敏度为40.0%,特异度为52.9%,阳性预测值为73.3%,阴性预测值为22.0%,准确度为43.1%。结论使用高质量的痰标本可以显著提升PCR检测肺结核的表现,特别是灵敏度和阳性预测值。

Influence of Sputum Quality on PCR Technology for Diagnosis of Pulmonary Tuberculosis
