宫腔镜下输卵管插管疏通术在输卵管堵塞中的疗效观察

目的:探讨宫腔镜下输卵管插管疏通术在治疗输卵管堵塞中的临床疗效。方法:选择2010年6月～2011年3月入该院妇产科治疗的62例输卵管堵塞患者,随机分为观察组34例和对照组28例,观察组患者行宫腔镜下输卵管疏通术,对照组患者行普通输卵管疏通术,比较两组患者治疗前后输卵管通畅情况,和术后1年内患者妊娠的情况,并分析患者不孕年限与疗效的关系。结果:治疗前两组患者输卵管堵塞和部分堵塞的数量均无统计学差异,治疗后较治疗前均显著改善,且观察组的疗效显著优于对照组;随着不孕年限的增加治疗效果越差,1～5年不孕者治疗后通畅率为76.92%,6～10年不孕者为53.34%,>10年者通畅率仅为16.67%;治疗后1年内观察组宫内妊娠23例,占67.65%,显著高于对照组12例(42.86%),观察组异位妊娠1例(2.94%),显著低于对照组4例(14.29%),P<0.05。结论:宫腔镜下输卵管插管疏通术治疗输卵管堵塞性不孕的临床效果良好,安全性高。

The observation on the efficacy of fallopian tube intubation deroppilation under hysteroscopic in tubal obstruction

Objective:To explore the clinical efficacy of fallopian tube intubation deroppilation under hysteroscopic in tubal obstruction.Methods:A total of 62 patients with tubal obstruction from June 2010 to March 2011 in our hospital were randomly divided into observation group(34 cases) and control group(28 cases).The observation group was taken with fallopian tube intubation deroppilation under hysteroscopic,and the control group was given with common intubation operation.The state of fallopian tube before and after treatment and the pregnancy after one year of suergery were compared in the two groups,then the relationship between the efficacy and infertility age was analyzed.Results:There was no significant difference in the number of blocked and partially blocked of fallopian tubes before treatment between the two groups,but they were significantly improved after treatment compared with before treatment,and the efficacy of the observation group was significantly better than the control group.With the increase of the infertility years,the treatment effect was worse,the patency rate after treatment was 76.92 % for the patients who were infertility 1~5 years,53.34% for 6~10 years,and only 16.67% for more than 10 years.There were 23 cases(67.65%) who had intrauterine pregnancy after treatment 1 year in the observation group,it was significantly higher than that in control group(12 cases,42.86%).There was 1 case(2.94%) who had ectopic pregnancy in the observation group,it was significantly lower than that in the control group(4 cases,14.29%),P<0.05.Conclusion:The clinical effect of treating tubal obstruction infertility by fallopian tube intubation deroppilation under hysteroscopy is good and safe.
