血管回声跟踪技术评价尿毒症患者颈总动脉弹性与内中膜厚度相关性的临床研究

目的探讨应用血管回声跟踪技术（echo-tracking,ET）定量评价尿毒症患者颈总动脉弹性功能与内中膜厚度（IMT）相关性的临床应用价值。方法选择尿毒症患者36例作为尿毒症组,正常体检者36例作为对照组,在ET模式下测量颈总动脉弹性指标,参数包括硬度指数（β）、压力应变弹性系数（EP）、血管顺应性（AC）及脉搏波传导速度（PWVβ）,同时测量颈总动脉IMT值。结果尿毒症组β、EP、PWVβ值均显著高于对照组（t=7.74、7.325、8.341,均P〈0.01）,AC低于对照组,但差异无统计学意义（t=-4.580,P=0.294）。尿毒症组的颈总动脉弹性参数β、EP、PWVβ分别与IMT呈正相关（r=0.744、0.883、0.883,均P〈0.01）,AC与IMT呈负相关（r=-0.714,P〈0.01）。结论 IMT增加与颈总动脉弹性降低呈显著正相关,IMT是反映尿毒症患者颈总动脉弹性功能早期改变的重要形态学指标。

Clinical study on correlation between carotid artery elasticity and carotid intima-media thickness in patients with uremia by echo-tracking evaluatation

Objective To assess the correlation of carotid artery elastic parameters and carotid intima-media thickness（IMT） in patients with uremia by using echo-tracking（ET）.Methods A total of 36 patients with uremia and 36 control subjects were enrolled in the uremia group and control group.In the ET mode,quantitative parameters of stiffness index（β）,pressure-strain elasticity modulus（EP）,arterial compliance（AC） and one-point pulse wave velocity（PWVβ） were measured,and carotid IMT was measured either.Results β,EP,PWVβ of the uremic group were significantly higher than those of the control group（t = 7.74,7.325,8.341,P 0.01） respectively,while AC was lower than that of the control group,but the difference was not statistically significant（t =-4.580,P = 0.294）.IMT of the uremic patients was positively correlated with β,EP,PWVβ（r = 0.744,0.883,0.883,P 0.01） respectively,which was negatively correlated with AC（r =-0.714,P 0.01）.Conclusion The decreased carotid artery elastic parameters are significantly in line with the increased IMT in the study group.IMT is an important morphological index which reflects the early change of carotid artery elastic function in uremic patients.
