改良超长降调节方案在卵巢低反应患者体外受精-胚胎移植中的应用

目的:探讨改良超长降调节方案对卵巢低反应患者体外受精-胚胎移植结局的影响。方法:回顾性分析华中科技大学同济医学院附属同济医院生殖医学中心86例行体外受精-胚胎移植术的卵巢低反应患者,其中33例采用短方案(A组),53例采用改良超长降调节方案(B组)。比较两组的超排卵天数及Gn用量、HCG日血清激素水平、获卵数、优质胚胎率、临床妊娠率和周期取消率等。结果:两组Gn启动剂量、HCG日E2值、HCG日子宫内膜厚度、获卵数及移植胚胎数比较无统计学差异(P>0.05);优质胚胎率及周期取消率,B组较A组高,但无统计学差异(P>0.05);Gn天数B组较A组长(P<0.05);临床妊娠率B组较A组高(P<0.05)。结论:改良超长降调节方案对于卵巢低反应患者是一种有效的促排卵方案。

Application of modified super-long down-regulation protocol in poor ovarian responders undergoing IVF-ET

Objective:To detect the affection of the modified super-long down-regulation protocol applied in IVF-ET for patients with poor ovarian response.Methods:The clinical data of 86 patients undergoing IVF-ET with poor ovarian response were analyzed retrospectively.Among them,33 cases(Group A) accepted short-protocol,53 cases(Group B) accepted modified super-long down-regulation protocol.The duration of stimulation,total amps of gonadotropin,estradiol level on HCG day,number of oocytes retrieved,rate of high quality embryo,clinical pregnancy rate per transfer,rate of cycle cancellation were compared between the two groups.Results:Between the two groups,there were no significant differences in the starting dose of Gn,estradiol level on HCG day,thickness of endometrium on HCG day,number of oocytes retrieved and transplanted embryos(P>0.05);the rate of high quality embryo and cycle cancellation in group B were higher than in group A,but with no significant differences(P>0.05),the day of Gn in group B were longer than group A,with significant difference(P<0.05);the clinical pregnancy rate of group B was statistically higher than that of group A(P<0.05).Conclusion:Modified super-long down-regulation protocol is an effectively protocol for poor ovarian responders.
