2009—2011年吉林大学第一医院住院患者抗菌药物应用分析

目的：了解我院住院患者抗菌药物的应用情况,为临床合理用药及科学管理提供依据。方法：采用回顾性方法,对2009—2011年我院住院患者抗菌药物销售金额、用药频度（DDDs）、限定日费用（DDC）以及病原菌耐药率进行统计与分析。结果：2009—2011年我院住院患者抗菌药物总销售金额呈逐年上升趋势,抗菌药物应用比例呈逐年下降趋势;第3代头孢菌素、头霉素类以及碳青霉烯类销售金额排序居前列;3年来头孢菌素类在销售金额与DDDs中一直占据较大比例;抗菌药物DDDs与病原菌耐药率呈正相关。结论：我院住院患者抗菌药物应用基本合理,但在抗菌药物使用过程中存在药物选择起点偏高、病原菌耐药率高等问题,医院应进一步规范抗菌药物的应用,尽量根据病原种类及细菌药物敏感性试验结果选用抗菌药物,使抗菌药物的应用更加安全、经济、有效。

Analysis of Utilization of Antibacterials in Jilin University First Hospital During 2009-2011

OBJECTIVE：To investigate the use of antibiotics in the inpatients of our hospital so as to provide reference for clinical rational use and scientific management of antibiotics.METHODS：In this retrospective study,the consumption sum,DDDs and the defined daily cost（DDC） of antibiotics as well as the antibiotic resistance in our hospital during 2009-2011 were analyzed statistically.RESULTS：The total consumption sum of antibiotics in our hospital during 2009-2011 increased year by year,yet its proportion in the total drug consumption declined year by year.The third generation cephalosporins,cephamycins and carbapenems took the lead on the consumption list.Over the 3 years,cephalosporin showed great proportion in both consumption sum and DDDs.The DDDs of the antibiotics was positively correlated with antibiotic resistance.CONCLUSION：The utilization of antibiotics in the inpatients of our hospital was basically rational yet there were still problems such as high starting point in drug choice and high antibiotic resistance rate etc.Therefore,the antibiotic use should be further standardized and the choice of antibiotics should be based on the types of pathogen and drug susceptibility results so as to facilitate safe,economic and effective antibiotic use.
