急性心肌梗死急诊介入治疗中血栓抽吸对心肌再灌注作用的研究

目的评价急性ST段抬高型心肌梗死（STEMI）患者急诊介入治疗（PCI）中血栓抽吸对心肌再灌注的作用。方法选择2010年4月~2011年5月我院收治的符合STEMI诊断标准的患者127例,分为抽吸组（52例）与直接PCI组（75例）,分析两组术后冠脉罪犯血管血流心肌梗死溶栓治疗分级（TIMI）水平、冠脉罪犯血管血流校正计帧数（CTFC）、左室射血分数（LVEF）及术后1个月内心脏不良事件发生率。结果两组患者TIMI血流分级水平差异无统计学意义（P〉0.05）,而CTFC差异有统计学意义（P〈0.05）,两组的LVEF水平差异无统计学意义（P〉0.05）,但术后1个月内心脏不良事件发生率血栓抽吸组显著优于直接PCI组（P〈0.05）。结论急诊PCI中进行血栓抽吸可有效地改善心肌再灌注,减少心脏不良事件发生。

Research of thrombus aspiration on myocardial reperfusion in patients with acute myocardial infarction by primary PCI

Objective To evaluate the effects of thrombus aspiration on myocardial reperfusion of patients with ST-segment elevation myocardial infarction（STEMI） by percutaneous coronary intervention（PCI）.Methods 127 patients with STEMI in our hospital from April 2010 to May 2011 were divided into aspiration group（52 cases） and directed PCI group（75 cases）.The level of TIMI flow grade and the values of number of CTFC were counted;the level of left ventricular ejection fraction（LVEF） and the rate of cardiac event were counted too.Results The difference of the level of TIMI flow grade between the two groups was not statistically significant（P〈0.05）,the difference of CTFC between the two groups was statistically significant（P〈0.05）,the difference of level of LVEF flow grade between the two groups was not statistically significant（P〈0.05）,the difference of the rate of cardiac event between the two groups was statistically significant（P〈0.05）.Conclusion Thrombus aspiration is an effective method,it can reduce thrombus burden,promptly recover coronary flow,it also can reduce rate of cardiac event.
