中西医结合预防髋周骨折术后深静脉血栓形成

目的探讨中西医结合预防髋周骨折术后下肢深静脉血栓（DVT）形成的临床疗效及安全性。方法根据入选标准对100例髋周骨折手术的患者分成两组。其中治疗组56例,联合采用基本预防、物理预防与药物（中西医结合）预防措施;对照组44例,仅采用基本预防的措施。主要的观察指标：①术后48 h的术口引流量;②术后血小板计数变化情况;③术前及术后双下肢彩色多普勒检查结果。结果治疗组DVT 5例（8.9%）,对照组DVT 14例（31.8%）,两组DVT发生率差异有统计学意义（P〈0.05）。两组患者术后48 h的术口引流量及术后血小板计数,差异无统计学意义（P〉0.05）。结论中西医结合（丹红注射液、低分子肝素联合间隙充气加压装置）能有效降低髋周骨折术后DVT的发生率,并发症少,在预防髋周骨折术后DVT的发生有着良好的社会及经济效益。

Integrative prevention of deep vein thrombosis around the hip fracture surgery

Objective To investigate the clinical efficacy and safety of integrative prevention of postoperative deep venous thrombosis（DVT） around the hip fracture surgery.Methods According to inclusion criteria,100 cases of hip fracture surgery patients were randomly divided into two groups.Prevention treatment group of 56 cases were given the joint use of basic prevention and physical prevention and medicines（integrative medicine） preventive measures;control group of 44 cases were only given basic preventive measures.Main outcome measures were that ： ①Surgery population drainage after 48 hours;② the changes of postoperative platelet count;③ the findings of preoperative and postoperative lower limbs color Doppler.Results Prevention treatment group of DVT were 5 cases（8.9%）,control group of DVT were 14 cases（31.8%）,two sets of DVT incidence difference were statistically significant（P〈0.05）.The difference of two groups of patients port drainage and postoperative platelet count 48 hours after surgery was not statistically significant（P〈0.05）.Conclusion Integrative medicine（Danhong Injection,low molecular weight heparin jointed with intermittent pneumatic compression device） can effectively reduce the incidence of postoperative DVT hip weeks fractures with fewer complications,it has good social and economic benefits in the prevention of the incidence of postoperative DVT hip weeks fractures.
