急诊冠状动脉内支架安置术治疗急性心肌梗死的临床效果

摘    要:①目的探讨急诊冠状动脉内支架安置术治疗急性心肌梗死的临床效果。②方法2001年3月~2003年12月,在我院就诊的急性心肌梗死病人160例,随机分为两组。治疗组78例,给予急诊冠状动脉内支架安置联合抗凝治疗;对照组82例,给予静脉溶栓治疗。住院4周左右行冠状动脉造影检查,了解冠状动脉再通情况。③结果治疗组在冠状动脉再通率、恢复TIMIⅢ级血流、心功能改善方面显著优于对照组(x^2=7.01~53.85,P〈0.01);而大出血发生率、病死率明显低于对照组(x^2=6.05、5.42,P〈0.05)。④结论急诊冠状动脉内支架安置术联合抗凝治疗急性心肌梗死是有效、安全的,可减少并发症的发生。

Abstract:Objective To evaluate the clinical effect of primary stent implantation for acute myocardial infarction (AMI). Methods During the period from March 2001 to December 2003, 160 patients with AMI admitted to our hospital were randomized to treated group (n=78) and control group (n=82). Patients in treated group were given primary stent implantation with associated anticoagulation; while in the control group, intravenous thrombolysis was administered. A selective coronary angiography was done in all patients. Results Compared with those in the control group, the patients in treated group showed increased coronary repatency, restoration of TIMI blood flow and cardiac function significantly (~2=7.01-53.85, P<0.01), and reduced massive hemorrhage and mortality significantly (~2=6.05,5.42; P<0.05). Conclusion Primary stent implantation with associated anticoagulation therapy is effective and safe, and can reduce complications.
