吉西他滨联合奥沙利铂治疗非小细胞肺癌60例

目的：观察吉西他滨加用奥沙利铂联合治疗老年晚期非小细胞肺癌（NSCLC）近期疗效和毒性反应。方法：60例老年晚期NSCLC接受治疗，方案为吉西他滨800～1000mg／nl^2静滴30min，第1天，第8天，奥沙利铂130mg／m^2静滴2h第1天，21天为1周期，化疗2周期后评价疗效。结果：60例患者累计完成176个周期，，其中CR1例（1．7％），PR24例（40％），PD20例（33．3％），总有效（PR＋CR）25例（41．7％），临床收益率（CR＋PR＋NC）40例（66．7％），中位缓解期为6．5个月。不良反应主要为骨髓抑制及神经系毒性。Ⅲ～Ⅳ级白细胞减少9例（15％），Ⅲ～Ⅳ级血小板减少8例（13．3％）；神经毒性发生率80％，均为轻度。结论：吉西他滨联合奥沙利铂治疗老年晚期非小细胞肺癌有效率高，不良反应可以耐受。

Gemcitabine Combined Oxaliplatin on Treatment of 60 Cases of Non-small Cell Lung Cancer

Objective ： To observe the clinical efficacy and toxicity of gemcitabine combined with oxaliplatin chemotherapy on elderly advanced non-small cell lung cancer（ NSCLC ） patients. Method： There were 60 patients of elderly advanced non-small cell lung cancer（NSCLC） who were treated with Gemcitabine 800 - 1000mg/m^2 on the 1 st and 8th days , and followed by Oxaliplatin 130mg/m2 on the 1st day. One cycle was 21 days. After 2 cycles the efficacy were evaluated. Result： A total of 60 patients were received 176 chemotherapy cycles, achieving CR 1 （1.7%） , PR 24 （40%）, NC15 （25 % ）, PD20 （33.3 % ） were achieved. Overall response rate （ PR ＋ CR） was 25 （41.7%） ,clinical benefit rate （ CR ＋ PR ＋ NC） was 40 （ 66.7% ）median remission period 6.5 months. The main side effects were myelosuppression and neurotoxicity, with grade 3 to 4 leucocytopenia in 15 % （9/60） , grade 3 to 4 thrombocytopenia was 13.3% （8/60） ; and neurotoxicity was 80%. Conclusion： The combination chemotherapy with Gemeitabine and Oxaliplatin is effecive in treatment of elderly advanced NSCLC with acceptable toxicity.
